Medical complications of lung transplantation

Abstract: Many of the medical complications that arise are the direct consequence of the need to administer potent immunosuppressive agents, with their attendant risks of infection, malignancy and drug toxicity. This article will review the major medical complications, excluding allograft rejection, which may be encountered in the lung transplant recipient. Familiarity with, and vigilance for, these problems should facilitate earlier recognition, more expeditious intervention and more favourable outcomes. Eur Respir J 20… Show more

“…In lung transplant recipients receiving calcineurin-inhibitor agents, renal failure is an increasingly recognized morbidity. Deterioration in renal function typically ensues early (16,17), with 91% of the patients experiencing renal dysfunction by 6 months (16) and 5-8% ultimately requiring hemodialysis (17,18). We note that one-half of the vascular catheter associated bacteremias after the first posttransplant year in our study were dialysis catheter-related.…”

“…There is growing interest in determining the impact of lung transplantation on functional status and QOL in patients with advanced lung disease (7)(8)(9). Preliminary studies employing standardized QOL questionnaires have reported significant short-and intermediate-term improvements in various QOL measures following lung transplantation (10)(11)(12).…”

“…(2,3) Therefore, it is important to include data regarding nutritional status and body composition in the pretransplant evaluation, since these factors can be related to complications that affect quality of life and life expectancy of recipients, as well as contributing to the increase in morbidity and mortality. (2)(3)(4)(5)(6) In Brazil, there have been no studies including the assessment of different anthropometric measurements in the evaluation of the nutritional status of lung transplant candidates and relating these measurements to lung diseases at more advance stages. Therefore, the objective of the present study was to determine the pretransplant nutritional profile of lung transplant candidates at a public hospital specializing in cardiology and pulmonology.…”
