Medical Informatics Education Needs Information System Practicums in Health Care Settings

SummaryBackgroundHealth information systems (HIS) are one of the most important areas for biomedical and health informatics. In order to professionally deal with HIS well-educated informaticians are needed. Because of these reasons, in 2001 an international course has been established: The Frank – van Swieten Lectures on Strategic Information Management of Health Information Systems.ObjectivesReporting about the Frank – van Swieten Lectures and about our students‘ feedback on this course during the last 16 years. Summarizing our lessons learned and making recommendations for such international courses on HIS.MethodsThe basic concept of the Frank – van Swieten lectures is to teach the theoretical background in local lectures, to organize practical exercises on modelling sub-information systems of the respective local HIS and finally to conduct Joint Three Days as an international meeting were the resulting models are introduced and compared.ResultsDuring the last 16 years, the Universities of Amsterdam, Braunschweig, Heidelberg/Heilbronn, Leipzig as well as UMIT were involved in running this course. Overall, 517 students from these universities participated. Our students‘ feedback was clearly positive. The Joint Three Days of the Frank – van Swieten Lectures, where at the end of the course all students can meet, turned out to be an important component of this course. Based on the last 16 years, we recommend common teaching materials, agreement on equivalent clinical areas for the exercises, support of group building of international student groups, motivation of using a collaboration platform, ensuring quality management of the course, addressing different levels of knowledge of the students, and ensuring sufficient funding for joint activities.ConclusionsAlthough associated with considerable additional efforts, we can clearly recommend establishing such international courses on HIS, such as the Frank – van Swieten Lectures.

Management of information systems can be divided into strategic, tactical, and operational management.Strategic management of information systems deals with the information processing of a healthcare facility as a whole. It comprises the planning of the information system’s architecture and of the organization of information management. The tasks of strategic management are planning an information system and its architecture, directing its establishment and its operation, and monitoring its development and operation with respect to the planned objectives. The strategic information management plan represents the long-term planning of the information system of a healthcare facility.Tactical management of information systems deals with particular functions, application components, or physical data processing systems that are introduced, removed, or changed. Usually, these activities are done in the form of projects.Operational management of information systems is responsible for operating the components of the information system. It ensures the smooth operation of the systems in accordance with the strategic information management plan of the healthcare facility.IT governance deals with the organizational structures for decision-making in the management of information systems. Typical organizational structures include the role of a chief information officer who focuses on the strategic management of information systems, an information management board, and a department of information management in charge of the strategic, tactical, and operational management.Management of information systems must find a balance between often conflicting goals, for example, between homogeneity and heterogeneity of the information system architecture or between functional leanness and functional redundancy.

Publish-subscribe models provide a scalable solution for exchanging information in the distributed environment of public health care. However, existing health information systems that have adopted the publish-subscribe models in the literature to alleviate the problem of delay in information delivery and retrieval are limited to Windows-based mobile platforms which are no longer popular with the availability of competing platforms such as Android operating System (OS) and iOS. Hence, this research aims to design model of a content-based publish-subscribe information system to represent the functional and non-functional requirements of the domain knowledge within the field using Microsoft Visio and Unified Modeling Language (UML). This research addresses the challenge of delayed information delivery in obstetrics and gynecological healthcare by developing a content-based publish-subscribe information system. The system utilizes a modern, platform-independent approach and deliver relevant health information to women of reproductive age. The evaluation findings showed average SUS score of 74.35, indicating the system is acceptable for use, with a good adjective and marginal acceptability scale. The initial findings suggest the system improves user knowledge about reproductive women’s health issues. This has the potential to improve maternal health outcomes and reduce child and maternal mortality rates.
