Medical Services and Associated Costs Vary Widely Among Surgeons Treating Patients With Hand Osteoarthritis

Abstract: Background There are substantial variations in medical services that are difficult to explain based on differences in pathophysiology alone. The scale of variation and the number of people affected suggest substantial potential to lower healthcare costs with the reduction of practice variation. Our study assessed practice variation across three affiliated urban sites in one city in the United States and related healthcare costs following the diagnosis of hand osteoarthritis (OA) in patients. Questions/purposes… Show more

“…There are substantial unexplained geographical and surgeonto-surgeon variations in rates of surgery [1,2,19,21,29]. This study addressed variation in treatment recommendations between surgeons choosing treatment for themselves or for patients the same age and sex as themselves and their confidence level when making these decisions.…”

“…There is substantial unexplained geographical and surgeonto-surgeon variation in rates of surgery [1,2,19,21,29]. The variation mainly pertains to discretionary procedures rather than clinical decisions that are constrained to a narrow range of treatment options or urgent and emergent surgical needs [2,21].…”

“…The study of unexplained variation and how to reduce it contributed to improved quality, safety, and resourcefulness in other fields [5], and many see the same potential in medicine [1,2].…”

“…Recent research indicates, however, that the quality of these services in general is sub-optimal [16], and that especially people with HOA have poor access to recommended treatment both in primary and secondary care [17, 18]. While persons with hip or knee-OA often receive physiotherapy, few with HOA have seen an occupational therapist [19, 20]. Thus, the care of people with HOA is most frequently limited to general practitioner (GP) consultations, while those with severe CMC1-OA may be referred for surgical consultation.…”
