Medication Adherence in Patients With Glaucoma and Disability

Abstract: IMPORTANCE People with disabilities tend to have lower medication adherence. Glaucoma medication adherence has been scantly studied for people with disability. OBJECTIVE To determine whether disability leads to reduced glaucoma medication adherence and whether this decrease varies by type of disability. DESIGN, SETTING, AND PARTICIPANTSThis population-based case-control study enrolled individuals with glaucoma and without disability, who were followed up until they received disability certification. “…Dementia was indeed associated with lower glaucoma medication adherence in a recent report. 24 Finally, another contributing factor to high adherence, in this study and all studies reporting electronic monitoring of medication, is the direct incentive effect of using a connected monitoring device for the patient, as the patient is aware of being followed-up in real time. This aspect, most probably variable among patients and time, may represent an intrinsic lever of interest to improve adherence level compared to real day life.…”

“…These intracranial tumors usually arise in the pineal or sellar region and are referred to as trilateral retinoblastoma when associated with preceding or concurrent retinal tumors. [1][2][3] Despite these associations, trilateral retinoblastoma is thought to develop independently of intraocular retinoblastoma. 4 We report a patient with de novo, mosaic retinoblastoma with a pineoblastoma as the primary tumor without ocular involvement.…”

“…The authors point out that this does not ensure that eye drops were instilled. 1 However, this proxy measure of adherence may not guarantee that eye drops were administered correctly by individuals or their caregivers; previous studies 5 have explored this and noted problems with drop administration and inaccurate perceptions of patients' own abilities despite seemingly high levels of adherence.…”

“…To the Editor We read with interest the recent study published in JAMA Ophthalmology by Hou and Pu on the association of disability with glaucoma medication adherence. This objective seems important as it introduces targets for future policies aiming to improve glaucoma outcomes.…”

“…First, the authors included patients with glaucoma from as early as 1997, while patients with disability onset were only included from 2014. Cases were not matched for length of glaucoma diagnosis; thus, variations between individuals in this parameter may have affected preexisting levels of adherence prior to the index dates.…”
