Medication adherence in women with epilepsy who are planning pregnancy

Abstract: The 75% compliance rate with an electronic diary suggests that it may be useful to track medication adherence in future studies and in the clinical setting. In those who tracked, the observed medication adherence rate was considerably higher than the 75% adherence rate seen in previous epilepsy studies. This might be explained in part by selection bias, but may also result from properties of the diary itself (daily reminders, real time feedback given to the provider). Women reported a higher rate of adherence … Show more

“…This study was a multicenter, observational study that allowed frequent sampling prior to and early in pregnancy. All women were trained and instructed to track their medication adherence, seizures, and menstrual flow daily using a customized mobile application (the WEPOD app), designed by Irody (Boston, MA), along with the cloud data system (WEPOD diary) . Participants’ menstrual cycles were monitored, and a pregnancy test was administered on day 35 if no menses occurred.…”

“…Although it is known that LTG clearance and estradiol concentrations increase throughout pregnancy, most studies have focused on the second and third trimester, as it is challenging to enroll pregnant women prior to this. This study prospectively followed women who were receiving LTG and planning to become pregnant, enabling collection of estradiol and LTG concentrations from preconception through 13 weeks gestational age. The objectives of our study were to determine how early LTG‐CL/F increases and quantify its relationship to estradiol concentrations and to gestational week during early pregnancy in women with epilepsy on LTG and no interacting concomitant medications.…”

“…Although numerous generic medication management apps are available, the use of customized apps for specific patient groups is only beginning to be developed and tested [911]. Promising results have been reported from studies involving patients with hypertension [12], epilepsy [13], and HIV infection [14] using medication apps to enhance their medication adherence. Regarding renal transplantation, several qualitative studies were recently published that evaluated the perceived benefits of an app for improving adherence [15,16].…”

“…Many PWE use mobile and online health (mHealth) solutions to monitor their disease. These mHealth applications take many forms, including alerting devices, therapy devices, and mobile self‐management apps . Unfortunately, as various mHealth solutions have arisen independently, they lack common standards.…”

“…These mHealth applications take many forms, including alerting devices, [3][4][5][6] therapy devices, [7][8][9] and mobile self-management apps. [10][11][12][13][14] Unfortunately, as various mHealth solutions have arisen independently, they lack common standards. As a result, systems are often not interoperable, and PWE who use multiple tools or transition between tools are required to enter information for each tool separately and redundantly.…”
