Medication-induced oesophageal injury leading to broncho-oesophageal fistula

Abstract: SummaryMedication-induced oesophageal injury is one of the least recognised side-eVects of oral medication and, in contrast to other oesophageal pathologies, is rarely considered in the diVerential diagnosis of chest pain. We describe a case of medicationinduced oesophageal injury with a rare complication in which the diagnosis was not considered until the characteristic features were demonstrated at endoscopy.

“…Several reports, either as sole cases or as small cohorts of patients, describe broncho-oesophageal communications as the result of malignancy, 6 inflammation, 7–11 trauma, 12,13 iatrogenic procedures 14–16 or drug-induced oesophageal injury. 17 Graph 1 demonstrates a schematic classification of BOFs according to their aetiology.…”

“…6 Additionally, vascular compromise of the conduit may predispose to fistula formation. 7 Treatments for bronchoesophageal fistulas have included the application of sealants such as silver nitrate and sodium hydroxide to the esophageal and bronchial ostia of the fistula. 8 It is possible that such a procedure may also work for colobronchial fistulas.…”

“…Not surprisingly, hemorrhage, which may be severe 19 , is a common complication of these esophageal ulcers, especially when compared with other medication related esophagitis. Broncho-esophageal fistula has also been reported 20 . Notably, it is over-the-counter use of NSAIDs that is most commonly associated with injury 21 , in keeping with their more commonly used venue.…”
