Purpose Follow-up and immunosuppressive medication (ISM) adherence are both important for kidney transplant recipients postoperatively and whether follow-up factors affect the ISM adherence remains unclear. The aim of this study was to examine the relationship between follow-up factors and ISM adherence, and the factors associated with ISM adherence. Patients and Methods An internet-based cross-sectional survey was conducted in a single kidney transplant center in China. The participants completed the internet-based questionnaire and the Basel Assessment of Adherence to Immunosuppressive Medication Scale (BAASIS©) from January 12 to January 26, 2021. Results Finally, 288 (66.7%) participants responded to this survey. The percentage of full adherence to immunosuppressant was 51.7% (149/288), with 33.3% of the participants reporting a problem in timing dimension. We found that follow-up with a fixed doctor was significantly positive to good adherence (OR=2.124, 95% CI=1.111–4.062, P =0.023) after analyzing the survey data. Time since kidney transplantation and number of non-immunosuppressants were both associated with immunosuppressant adherence. No significant difference was found regarding the effect of the follow-up adherence on ISM adherence. Conclusion Our study demonstrated an insufficient prevalence of adherence to immunosuppressant in Chinese renal transplant recipients and revealed that follow-up with a fixed doctor may be a way to improve the patients’ ISM adherence. This anonymous internet-based survey provides valuable insight into the actual adherence rate, factors associated with non-adherence, and situations that may improve medication-taking.

Medication non-adherence (MNA) is a major issue in kidney transplantation and it is associated with increased risk of rejection, allograft loss, patients’ death, and higher healthcare costs. Despite its crucial importance, it is still unclear what are the best strategies to diagnose, prevent, and treat MNA. MNA can be intentional (deliberate refusal of adhere to a prescribed medication) or unintentional (non-deliberate missing the prescribed medication). Its diagnosis may rely on direct methods, aiming at measuring drug ingestions, or indirect methods, which analyze the habits of patients to check their adherence to correct drug dose (taking adherence) and intervals (time adherence). Identifying individual risk factors for MNA may provide the basis for a personalized approach to the treatment of MNA. Randomized control trials performed so far tested a combination of strategies, such as enhancing medication adherence through the involvement of healthcare personnel involved in drug distribution, the use of electronic reminders, therapy simplification, or various multidisciplinary approaches to maximize the correction of individual risk factors. Although most of these approaches reduced MNA in the short-term, the long-term effects on MNA, and, more importantly, on clinical outcomes remain unclear. In this review, we provide a critical appraisal of traditional and newer methods for detecting, preventing, and treating non-adherence to immunosuppression after kidney transplantation from the perspective of the practicing physician.

Tyrimo tikslas − įvertinti pacientų po inkstų transplantacijos gydymo rekomendacijų laikymąsi ir nustatyti veiksnius, susijusius su režimo pažeidimais. Metodika. Sudaryta anoniminė įsivertinimo anketa apie vaistų vartojimą, socialinius veiksnius, įsitikinimus, informuotumą, nepageidaujamus reiškinius, lėtines ligas, priminimo priemonių naudojimą ir reguliarų atvykimą pas nefrologą. Duomenųanalizei naudojama statistikos programųpaketo SPSS 27,0 versija. Rezultatai. Iš 56 tiriamųjų: 55,4 proc. vyrai, 44,6 proc. moterys, amžius 53±12 metų. 98 proc. tiriamųjų laiku išgeria imunosupresantus, 89 proc. – kitus vaistus, 89 proc. nėra praleidę ar pakeitę vaistų dozių, 97,5 proc. nėra sumažinę vaistų dozės dėl šalutinių reiškinių, 91 proc. – dėl nuotaikų kaitos. Visų rekomendacijų griežtai laikėsi tik 39,3 proc. tiriamųjų, patikimų sąsajų tarp vaistų vartojimo ir tirtų rizikos veiksnių negauta. Teisingas vaistų vartojimas net 68 proc. pacientų susijęs su varginančiais šalutiniais reiškiniais. Išvados. Mažiau nei pusė pacientų po inksto transplantacijos pripažino tinkamai besilaikantys gydymo režimo. Pacientų įsitikinimai, socialiniai veiksniai ir įpročiai nebuvo susiję su režimo pažeidimais.
