Background: Infection is the most common complication in burn-injured patients and is believed to contribute to the hypertrophic scarring frequently observed in such injury. Pseudomonas aeruginosa is a common pathogen in burn wound infection. We examined the effect of local probiotic therapy with Lactobacillus plantarum on the severity of the scarring following burn wounding and infection with P. aeruginosa in a rabbit model. Methods: Full-thickness burn wounds were inoculated with control vehicle or L. plantarum; wounds were then challenged with bioluminescent P. aeruginosa. The time course of the ensuing infection was monitored by quantification of the emitted light. After allowing wounds to contract to near completion, they were harvested and analyzed for markers of scar formation. Results: Application of L. plantarum curtailed both the severity and the length of the pseudomonal infection. Probiotic therapy significantly reduced both Type I collagen mRNA concentrations and total collagen protein accumulation in infected wounds, consistent with reduced scarring. Surprisingly, the probiotic showed a nearly equivalent effect in uninfected wounds. Masson's trichrome staining confirmed these findings histologically. Conclusions: Lactobacillus plantarum shows exciting potential as a therapeutic agent to both counteract burn wound infection and to alleviate scarring even in the absence of infection.

Background/Aim. Medico-legal aspect of physical disfigurements has been subject of few scientific researches. The aim of this study was to establish a standardized instrument for forensic evaluation of all kinds of physical disfigurements, which has not existed until now. Methods. The research was conducted in 3 phases: the first one-drafting a list of disfigurements; the second phase-evaluation of the draft of the disfigurement list provided by 11 experts (plastic surgery lecturers) by the use of the Delphy method; the third one during which 51 medical doctors, members of Serbian Association of Plastic, Reconstructive and Aesthetic Surgery were evaluating the list of disfigurements. Results. Totally 176 individual physical disfigurements were described in the first phase of research. In the second phase, 217 disfigurements were established which were classified into 5 degrees of severity 0-none/very small, 1small, 2-medium, 3-large, 4-very large disfigurements. In the third phase, 217 disfigurements were evaluated. Conclusion. The first 4-point ordinal scale was established, rating 217 disfigurements, which showed high level of compatibility in practice and which can be used as an instrument for standardization of disfigurements in giving of forensic expertise.

In the last decades, the elevated number of victims of traffic accidents, urban violence, sports-related trauma, occupational injuries, and professional-related errors increased the concern with the expert aspect, whether in the criminal or civil context, because of the sequelae associated with these injuries and trauma, also to the maxillofacial region. In this sense, this study aimed to translate into Brazilian Portuguese and culturally adapt for use in Brazil the Spanish instrument called Analysis of Aesthetic Damage Impression and Impact (AIPE) proposed by CoboPlana (2010), and to validate the instrument among dentists working in forensic dentistry. After translation and cultural adaptation, fake scars (moulage) were placed on the maxillofacial region of two models, a male and a female. Dentists specializing in forensic dentistry were selected to make the assessments. These students used the instrument, which consists of four tables, on a sequence of ten randomized images, showing models with and without facial scars. The intraclass correlation coefficient measured observer reliability to verify instrument validity. In general the method was satisfactory for one scar and excellent for two scars of the three repeated scars. Regarding intertester error, descriptive analysis of the results showed a variation coefficient of the scores of 40.12% when all the coefficients were considered, demonstrating that the scores were relatively homogeneous. The four tables translated into Brazilian Portuguese and culturally adapted for Brazil have the potential to increase the objectivity of aesthetic damage assessment.
