“…Some studies suggested an increase in acute mortality following TAVR in patients with significant MR,9 16 18 20 26 but others failed to demonstrate such association 8 17 19. This pooled analysis including patients from TAVR national registries and randomised trials showed that moderate–severe MR was associated with increased 30-day mortality.…”

“…Thus, we chose a 23‐mm Sapien XT for the aortic valve and a 29 mm for the mitral valve. Because there was a significant discrepancy between the internal diameter of the aortic stented valve and the size of the Sapien valve we found higher postoperative transaortic gradients than a conventional TAVR that is usually around 10 mm Hg . However, this slightly higher gradient doesn't have a significant clinical impact on patient's hemodynamic performance or on functional class.…”

“…29,[36][37][38] The proportion of patients having TF versus TA varies widely, ranging from 30% to 80% TF, and most clinicians agree that patients undergoing TA are a higher-risk population. Despite this situation, an early report by Bleiziffer and colleagues 40 compared a large series of 153 TF with 50 TA patients who were selected based only on access and reported no difference in 30-day mortality (11.2% TF vs 8.3% TA, P 5 .918) and no stroke in TA versus 7% in TF (P 5 .041).…”
