“…Case series in healthy children (Ivanenko et al 2003) or those with developmental disorders (Miyamoto et al 1999;Niederhofer et al 2003) suggest that melatonin has been effective for sleep disorders, while randomised controlled studies in typically-developing children have also shown benefits, particularly with sleep onset (Smits et al 2001(Smits et al , 2003. However, very few studies have examined its effectiveness for children with developmental disorders.…”

“…The etiology of ADHD-related chronic sleep-onset insomnia is unknown. In a series of studies on childhood chronic sleep-onset insomnia, we found many ADHD children with characteristics of a delayed endogenous circadian pacemaker, such as a delayed sleep onset, dim light melatonin onset (DLMO), and wake-up time Smits et al, 2001). The endogenous circadian pacemaker is the hypothalamic suprachiasmatic nucleus that endogeneously generates a near 24 h rhythm, and entrains to light-dark input ascending from the retina.…”

“…Melatonin is only a mild hypnotic, and is therefore most useful in children with a wide range of circadian rhythm disturbances (including jet lag, blindness, central nervous system pathology, and, interestingly, ADHD) [52]. Melatonin has been studied in pediatric populations and has demonstrated safety and efficacy at shortening sleep-onset latency [53][54][55]. It is important to note that melatonin is a naturally occurring compound and in the United States is therefore not regulated by the FDA.…”
