Melioidosis in a Returned Traveller

Abstract: Melioidosis is an infection endemic to Southeast Asia and Northern Australia, and is associated with significant morbidity and mortality. The present report describes a case of chronic melioidosis in a returning traveller from the Philippines. Clinical suspicion of this illness is warranted in individuals with a history of travel to endemic regions. Safety in handling clinical specimens is paramount because laboratory transmission has been described.

“…The number of case reports of melioidosis from various countries is continually increasing in the literature, as awareness of the disease is improving worldwide. Several recent cases of travel-associated melioidosis have also been documented in individuals returning to regions where the disease is nonendemic from Southeast Asia, Africa, and the Caribbean ( 4 12 ).…”

“…In addition, ProMED may be useful as a source of information for travel medicine practice. 10 Melioidosis is well described amongst both short- and long-term visitors to endemic areas and so needs to be considered in the evaluation and treatment of patients with sepsis or other febrile illnesses returning from endemic countries, 34,35 particularly from Southeast Asia and Northern Australia as has been seen in this study. Melioidosis should be considered in anyone with compatible clinical manifestations, even in the absence of apparent risk of exposure to B. pseudomallei .…”

“…The clinical manifestations of melioidosis vary from benign localized abscesses to severe pneumonia, to acute fulminating septicemia with multiple abscesses, often leading to death [ 1 , 2 ]. Pericardial effusion is a rare presentation of melioidosis and has previously been reported in a series of case reports [ 3 , 4 ]. Constrictive pericarditis is an uncommon post-inflammatory disorder characterized by a variably thickened, fibrotic, and frequently calcified, pericardium.…”

“…Melioidosis is a tropical infection that most commonly occurs in South East Asia, India and northern Australia [ 2 ]. Because of the availability of long-distance travel, sporadic cases of melioidosis have been reported, including a case in a Swiss traveler returning home after visiting Thailand, which is an endemic area for Burkholderia pseudomallei [ 3 ].…”
