Melioidosis: Recrudescence with a Strain Resistant to Multiple Antimicrobials1

Abstract: We describe a recent case of melioidosis presenting classically in a young Vietnam veteran. The case demonstrates extensive antibiotic resistance in this isolate and failure of ceftriaxone, despite its in vitro efficacy. Other multiple antimicrobial regimens also failed. Melioidosis remains a concern in patients who have previously lived in endemic areas. A high level of diagnostic suspicion is required, especially in patients with a febrile illness or tuberculosis-like presentations. Early detection of Pseudo… Show more

“…Although tetracycline or trimethoprim-sulfamethoxazole has been suggested as primary therapy for melioidosis, no satisfactory treatment regimens have been established (15). P. pseudomallei is not only resistant to therapy with many antibiotics but clinically may fail to respond to those drugs to which it is susceptible in vitro (13,26). The present case reveals good correlation between clinical responsiveness and the in vitro results with amikacin.…”

“…This therapy appeared to be effective and resulted in defervescence. Although cloudy pneumonia of both lungs persisted as evidenced by chest radiograph, the patient was discharged on 26 October. Because of positive microhemagglutination assay for Treponema pallidum, she was treated with penicillin G benzathine three times before discharge (2.4 million U per week).…”

“…On examination she was febrile with a temperature of 39.4"C; a full blood count demonstrated a pancytopenia with a white cell count of 3.0 X IOy/L and an erythrocyte sedimentation rate (ESR) of 18 revealed a right psoas abscess and blood cultures grew B. pseudomullei.…”
