Membranous glomerulonephritis in chronic lymphocytic leukemia

Abstract: We present a case of chronic lymphocytic leukemia associated with nephrotic syndrome. Renal biopsy revealed membranous glomerulonephritis accompanied by interstitial monoclonal lymphocytic infiltration. Combination therapy with cyclophosphamide, vincristine, and prednisone (COP) was successful in inducing an effective response in chronic lymphocytic leukemia. Improvement of renal function and proteinuria was also observed. Am.

“…Early administration of penicillin is reported to prevent or ameliorate the severity of acute glomerulonephritis and at least one report suggests that APSGN patients that receive antibiotic treatment have a milder clinical course. 27,28 If infection is not apparent at the time of diagnosis, antibiotic treatment should be given anyway because positive cultures are sometimes obtained in apparently healthy patients and cross infection of household members and siblings of index cases is very high. 12 Although a 10-day course of oral antibiotic therapy with penicillin is recommended to limit the spread of the nephritogenic organisms, antibiotic therapy does not affect the natural history of glomerulonephritis.…”

“…Obstruction of the ureters, sepsis, hemolysis, and therapy-related toxicity are the most common causes of renal disease in patients with these diseases. 4 Glomerulonephritis with NS rarely occurs in patients with CLL and a variety of glomerular lesions have been observed. While the majority of patients with CLL-associated NS have been found to have MPGN, MN, a common cause of NS in adults and the most common glomerulonephritis in carcinomas, is rarely associated with CLL, despite the relative frequency of both diseases.…”

“…The production of autoantibodies that recognize renal epithelial cell structure as well as altered glomerular permeability caused by cytokines produced by T-cells are postulated to participate in the genesis of MN. 4 Interstitial lymphocytic infiltration occurs in 50-60% of patients with CLL. Recognition of neoplastic nature of small infiltrates may be at times challenging.…”
