Background: Mental Health(MH) is among the main determinants of community well-being and a remarkable portion of global burden of diseases is pertaining to mental disorders. Despite this fact, little attention has been given to the provision of adequate and appropriate MH services and improving equitable access to these services in some parts of the world especially in low-and middleincome countries(LMICs). This review was aimed to identify key barriers to the provision and utilization of MH services in LMICs. Methods: We carried out a systematic and comprehensive search on 7 important online databases for English-language literature on key barriers to the provision and utilization of MH services in LMICs from Jan 2000 to Nov 2019. Arksey and O'Malley instructor was used in 5 steps to implement scope study. The extracted materials were tabulated and synthesized using a qualitative content analysis approach. Results: Three main themes were developed regarding the barriers to the provision of MH services in LMICs including resource and administrative barriers, information and knowledge obstacles, as well as policy and legislation barriers. Also four main themes emerged about barriers to the utilization of MH services including attitudinal, structural, knowledge, and treatment-related barriers. Conclusion: Equitable access to MH services in LMICs is under influence of many barriers in both provision and utilization sides. In order to alleviate these problems, health systems in LMICs could adopt some strategies including integration of MH policy into general health policy, improvement of public MH knowledge regarding the MH and its related issues, developing anti-stigma programs, reallocation of health resources toward high-priority MH needs, developing community-based insurance, as well as integration of MH services into all levels of health-care systems. Further studies are also required for determining relative importance of the barriers and their related solutions based on the national context of each country.

Background We aimed to assess the effects of body mass index (BMI), activities of daily living (ADL), and subjective well-being (SWB) on cognitive impairment and propose dynamic risk prediction models for aging cognitive decline. Methods We leveraged the Chinese Longitudinal Healthy Longevity Survey from 1998 to 2018. Cognitive status was measured using the Chinese Mini-Mental State Examination. We employed repeated measures correlation to assess associations, linear mixed-effect models to characterize the longitudinal changes, and Cox proportional hazard regression to model survival time. Dynamic predictive models were established based on the Bayesian joint model and deep learning approach named dynamic-DeepHit. Marginal structural Cox models were adopted to control for time-varying confounding factors and assess effect sizes. Results ADL, SWB, and BMI showed protective effects on cognitive impairment after controlling observed confounding factors, with respective direct hazard ratios of 0.756 (0.741, 0.771), 0.912 (0.902, 0.921), and 0.919 (0.909, 0.929). Dynamic risk predictive models manifested high accuracy (best AUC = 0.89). ADL was endowed with the best predictive capability, although the combination of BMI, ADL, and SWB showed the most remarkable performance. Conclusions BMI, ADL, and SWB are protective factors for cognitive impairment. A dynamic prediction model using these indicators can efficiently identify vulnerable individuals with high accuracy. Important Systemic Factors for Improving Adolescent Mental Health Literacy

Objective: Mental health literacy is especially important during adolescence and youth, because the peak of the onset of mental disorders occurs in this period. The present study aimed to identify the effective systemic factors for improving Iranian adolescent mental health literacy from the Perspective of policymakers and experts. Method: This qualitative study was performed on 21 policymakers and health literacy/mental health experts since May 2020 to September 2020 in the workplace of the interviewees in Tehran. Purposive sampling (snowball method) was conducted based on experience, expertise, and the willingness to participate in an interview. All interviews were conducted with the presence of the interviewer at the interviewees' workplace in Tehran. The data were collected through semi-structured interviews and analyzed via the conventional content analysis method. Results: Five themes were extracted as systemic factors for improving adolescent mental health literacy. Themes were “mental health literacy training”, “integration and coordination of stakeholder organizations”, “resources and facilities”, “continuous assessment” and “provision of information.” Conclusion: Before policy-making and planning to enhance adolescents' knowledge and awareness of mental health issues, it is necessary to attract policymakers’ attention to the macro level and identify direct and indirect strategies for the correct implementation of policies adopted in this field.
