Objective: To determine the diagnostic efficacy of haematological indices for the diagnosis of beta thalassemia trait taking haemoglobin electrophoresis as a gold standard. Study Design: Cross sectional study. Place and Duration of Study: Department of Haematology Combined Military Hospital Lahore, from Aug to Dec 2019. Methodology: Two hundred and six anaemic patients 20-70 years of age of both genders were included in the study. Blood was collected from all the patients and complete blood count were generated through automated haematology analyser Sysmex KX-21. Based on the complete blood count parameters like mean cell volume, red blood cell count, red cell distribution width and mean cell haemoglobinthe haematological indices such as Mentzer, Ricerca, mean density of Hb/litre of blood, mean cell Hb density and red cell distributon width indices were calculated. The haemoglobin A2 band of >3.5% on haemoglobin electrophoresis was taken as cut off for Beta thalassemia trait. Results: Out of the 206 blood samples analysed, 120 (58%) were labelled as Beta thalassemia trait after Hb electrophoresis and 86 cases had anemia due to other causes. Mentzer index established the highest sensitivity of 92.5% whereas RICERCA index established the highest specificity of 94.1%. Mentzer and red cell distribution width indexhad highest diagnostic efficacy value up to 90.3%. Conclusion: We conclude that Mentzer and red cell distribution width index showed the highest diagnostic significance. Background Hemoglobinopathies represent a set of inherited red blood cell (RBCs) disorders, characterized by abnormal hemoglobin molecule. They include qualitative and quantitative hemoglobinopathies, with a structurally abnormal globin chain in the first and defective production in the later. This study assessed, for the first time, the significance of the mathematically calculated RBC indices to identify patients with hemoglobinopathies from normal subjects or iron deficiency anemia (IDA) and to differentiate various types of hemoglobinopathies from each other. Subjects and methods The study was a comparative hospital based and included 167 participants with hemoglobinopathies (group 1) and 49 participants with IDA (group 2) as an active comparator. Another 50 healthy volunteers (group 3) were also included. All participants were subjected to medical history, clinical examination, CBC, and HPLC. Next, 10 RBC indices were mathematically calculated from the CBC for each participant. Results Gender analysis shows that females represent 36.8% in thalassemia group, 42% in sickle cell disease (SCD) group, and 71.4% in IDA group. The receiver operating characteristic (ROC) curve shows that Ehsani index (EI) is the most reliable screening tool for thalassemics because of showing the highest Youden index and specificity of 41.88% and 88.89%, respectively, followed by Shine and Lal index (SL), with Youden index (YI) value, specificity, and sensitivity equal to 39.78%, 69.70%, and 70.09%, respectively. Similar results were found for IDA. For SCD, SL index is the most suitable screening tool. In conclusion, the mathematically calculated RBC indices are available, cheap, reliable, and sensitive tools for screening patients with hemoglobinopathies.

Thalassemia is a disease that attacks the hemoglobin gene, causing disturbances in erythropoiesis. Anemia is a condition in which the hemoglobin (Hb) concentration and the number of erythrocytes are below normal limits. The high number of anemia and thalassemia causes the need for early detection in order to prevent adverse effects in the future. Research objectives: to determine nutritional status, anemia status, and application of the Mentzer Index for SMK Muhammadiyah II Moyudan students. Methods: This study is a descriptive study with a cross-sectional design. The research subjects were first graders at SMK Muhammadiyah 2 Moyudan. Venous blood samples were examined at the Clinical Pathology Laboratory of FK-KMK UGM using the Sysmex XN-1000 tool on November 25, 2021. Data analysis used IBM SPSS Version 26 and Microsoft Office 365. Results: A total of 53 subjects participated in this study. A total of 47 (89%) respondents had normal hemoglobin values, and 6 (11%) respondents suffered from anemia. 31 (58.5%) respondents have normal nutrition, 15 (28%) respondents have less nutrition, 7 (13%) have excess nutrition, and 29 (54.7%) respondents have normal status. A total of 2 (3.8%) respondents showed Thalassemia. Conclusion: There are two students of SMK Muhammadiyah 2 Moyudan diagnosed with thalassemia and five students diagnosed with anemia. No statistically significant difference exists between Anemia Status in Adolescents at SMK Muhammadiyah 2 Moyudan and Nutritional Status. Further study is needed in wider and more diverse populations to analyze the influence of nutritional conditions on anemia in adolescents.
