Resurfacing of the hip with metal-on-metal devices is becoming more and more popular in younger patients. As the preserved femoral head interferes with the preparation of the socket the standard approach needs longer incisions. This paper describes a lesser invasive technique in hip resurfacing with the posterior approach. Compared to the posterior and lateral standard techniques, the minimal dorsal approach needs an incision of around 10 cm, depending on the anatomical conditions, shortens the duration of the operation, decreases blood loss and the incidence of complications and has high patient satisfaction. This technique therefore is recommended to surgeons who are familiar with the procedure of resurfacing with the dorsal approach.
