Penile squamous cell carcinoma (pSCC) is a rare cancer with several known risk factors. High-risk HPVs are considered to be responsible for about 50% of cases 1 and it is established that concomitant HIV infection accelerates disease progression. 2 Although significant advancements have been made in systemic tumor therapy, the prognosis of advanced pSCC remains inauspicious with poor quality of life.We report an extraordinary response of an HIV positive patient with advanced penile SCC, treated with cemiplimab.F I G U R E 1 A, Clinical evaluation at different time points showing, from left to the right, painful ulcerated, and sclerotic lesion with serumpurulent secretion involving penis and scrotum, with purple infiltrated plaques on the pubis and enlarged lymph nodes. Second images shows partial regression after 1 month of treatment and third reveal total remission after 3 months. B, Contrast-enhanced computed tomography (ce-CT) acquired pre-therapy (B, middle row) showed inhomogeneous enhancement in penis (white arrow) and in pathological lymph nodes in the inguinal, iliac, and paracaval region (red circles). C, After epicystostomy and 12 weeks of treatment with cemiplimab, a further ce-CT scan demonstrated no pathological enhancement in penis (white arrow) and regression of the pathological lymph nodes (red circles)
