Metastatic Squamous Spindle Cell Carcinoma of the conjunctiva

Abstract: Conjunctival Squamous Cell Carcinoma (CSCC) is one of the important ocular surface tumors. It is thought to be more advanced in this part of the world such as in Saudi Arabia. The Squamous Spindle Cell Carcinoma (SSCC) is a more aggressive variant. A single case of spindle squamous cell carcinoma tumor-related death due to bone and lung metastasis has been reported in Sweden in 1995. We are reporting a similar case of SSCC of the conjunctiva with eventual death because of bilateral lung metastasis in our area.… Show more

“…The sarcomatous component is vimentin-positive with weak staining for cytokeratin/EMA. p63 and p40 are both useful markers with comparable sensitivity for epithelial differentiation in such cases, however p40 has greater specificity [12] . Histological feature to guide Immunohistochemistry (IHC) for sarcomatoid squamous cell carcinoma is the presence of spindle cell tumour, which may have continuity with surface epithelium.…”

“…Mortality from metastatic lung disease had been reported by Seregard and Kock and Alkatan et al . [ 5 11 ] In the present case of a rare, presumably more aggressive SSCC accompanied with both intraocular and intraorbital invasion, MMC was used as an adjuvant therapy with lesion soaking after tumor excision during surgery and topical eye drops was administered at clinical follow-up. Topical MMC has been documented effective without significant adverse effects.…”

“…In other large OSSN case series of 389 patients (57 SCC) [29] and 288 patients (62 SCC) [7], no intraocular invasion was reported. Variants of conjunctival SCC manifest more aggressively and have an increased likelihood of exhibiting intraocular invasion, namely the mucoepidermoid [16,30,31] and spindle cell variants [32,33,34] as well as patients displaying HIV positivity [35]. Rao and Font [30] reviewed 5 cases of mucoepidermoid carcinoma of the conjunctiva and found intraocular invasion in 2 cases and orbital extension in 3 cases.…”
