Metformin-Induced Generalized Fixed Drug Eruption With Cutaneous Hemophagocytosis

Abstract: Fixed drug eruption (FDE) consists of recurrent dusky-red to brownish macules or patches at the same sites after the readministration of the causative drug. It usually presents as a solitary lesion, but generalized eruptions have been described. The most frequently implied drugs are antibiotics, anticonvulsants, and analgesics. Only 2 cases due to metformin have been reported. Histopathologic features of FDE include vacuolar degeneration of the basal layer, necrotic keratinocytes, and superficial and deep peri… Show more

“…Erythroderma secondary to metformin is described as a very rare side effect (1 in 10,000 treated patients); the condition is more common with penicillins, sulfonamides or allopurinol [4, 5] . Treatment should include withdrawal of the precipitating drug, and administration of high-dose corticoids and PUVA.…”

“…The cases of FDEs resulting from metformin varied from a generalized macular eruption with cutaneous hemophagocytosis to discrete erythematous lesions on the palms and soles of the patient [21,22]. Each resolved with metformin discontinuation, and like others in this family of drug reactions, recurred in the same location when exposed to the same medication.…”
