Methadone for chronic non-cancer pain in adults

Abstract: BackgroundMethadone belongs to a class of analgesics known as opioids, that are considered the cornerstone of therapy for moderate-to-severe pain due to life-threatening illnesses; however, their use in chronic non-cancer pain (CNCP) is controversial. Methadone has many characteristics that di erentiate it from other opioids, which suggests that it may have a di erent e icacy and safety profile. ObjectivesTo assess the analgesic e ectiveness and safety of methadone in the treatment of CNCP. Search methodsWe id… Show more

“…Methadone has also been cited as a promising treatment for its NMDA antagonist action and it has been shown to be effective for NP that is resistant to conventional analgesics [54,55]. However, a recent Cochrane review stated that methadone use for non-cancerous pain is controversial [56,57]. Indeed, the evidence is sparse regarding the use of specific oral or transdermal opioids in the treatment of NP from SCI.…”

“…When combined with social services, MMT has been shown to decrease high‐risk behavior such as needle sharing, criminal activity and in some studies it has been linked to lower mortality . For chronic pain, methadone is a valuable, moderate cost, therapeutic alternative, especially when other opioids have failed as it has a lower propensity for patients to develop tolerance and there is NMDA receptor antagonism .…”

“…It is most commonly prescribed as a tablet for pain but can be administered IV. 103 Liquid formulation is the most common and cheapest dose strategy for methadone clinics, and tablet formulations are the most common prescription formulations in pain clinics. A majority of patients require 80 mg per day to 120 mg per day of methadone or more to achieve the desired effects, with lower doses shown to be typically less effective.…”
