The perspectives of Russian health care in the transition to personalized medicine are realized in the formation of an innovative digital health care system. Forming with the use of digital technologies the accessibility of health care provides enormous social advantages in regional conditions and assumes a qualitatively new level of regional health care system. The problems of the COVID-19 pandemic, requiring isolation, quarantine and social distancing and characterized by the formation of trends of social “atomization”, accelerated the dependence on digital health care, and served as a “catalyst for the development and implementation of a wide range of remote monitoring technologies in health care delivery”. However, the digitalization process is “objectively slow” in Russia. The research establishes a high differentiation of digitalization of regional health care systems in the Russian Federation. The implementation in regional health care of digital technologies, which have a high potential for improving the efficiency of the health care system, is limited by institutional barriers. The article presents a typology of institutional barriers to the digitalization of the regional health care system: on federal level [legal barriers, organizational and managerial (logistical) and technological barriers], regional level (financial barriers), level of medical organizations and users (medical staff and patients) [behavioral barriers, organizational and managerial (logistic) and technological barriers]. Modern health care is experiencing large-scale digital technology inclusion, which can radically change the health care system by determining the accessibility and quality of medical care. However, the provision of cost-effective and quality health care initiated by the regional health care digitalization process is limited by institutional barriers at the federal, regional, medical organization and user levels.

The research topic related to the healthcare system is relevant, as it is one of the key areas of social policy and the effective management of this system is important to ensure high-quality and affordable medical care to the population. Currently, Kazakhstan is facing a few challenges and changes that require updating and developing the theoretical foundations of management in the healthcare system. The purpose of this work to analyze and assess the current state and problems in the healthcare of Kazakhstan to develop and propose theoretical foundations of management that contribute to its more effective organization. Among the used methods were the analytical, statistical, functional, system analysis, deduction, synthesis and comparison method. The study determined the organizational of the healthcare in Kazakhstan, including the roles and functions of the main institutions and responsible for the management and provision of medical care. The quality and availability of medical care in Kazakhstan is investigated. Various theoretical concepts, models and approaches to healthcare management were studied in order to develop new theoretically sound management models that take into account the unique features of the healthcare system in the Republic of Kazakhstan. This study shows that the financing of the healthcare system is an important aspect for providing affordable medicine to all citizens of the Republic of Kazakhstan. In the course of the research within the framework of this article, scientific and practical recommendations were prepared for government agencies.

Improving efficiency is one of the important aspects in the health care system. One of the key issues today is the improving the quality of management institution of medical care to the population. The study of the satisfaction of all participants in the diagnostic and treatment process and their interaction during medical care process is very important aim. The satisfaction of patients and employees of a medical institution during the treatment and diagnostic process is influenced by many factors. Achieving the maximum level of satisfaction of patients and employees of a medical institution and the quality of the management of medical care is possible with appropriate indicators of the structure, process and result, but it depends strongly on the effective interaction between all elements both within one medical institution and at the level of the healthcare system. A search of literature was performed with databases Scopus, Web of Science, MedLine, The Cochrane Library, EMBASE, Global Health, CyberLeninka, RSCI. This review provides information on the factors affecting the satisfaction of participants in the diagnostic and treatment process and their interaction during providing the medical care. Both the satisfaction of employees and patients, and the successful interaction of all participants of the diagnostic and treatment process positively affect the efficiency in the healthcare system. The purpose of the presented work is to analyze experience regarding the factors affecting social efficiency, satisfaction and interaction of participants in the treatment and diagnostic process. According to the results of this analysis, the factors influencing the interaction of the parties of the treatment and diagnostic process, improving the quality of the management of medical care and efficiency in the healthcare system as a whole,were noted to be correlated to each other and could be managed by the development of special algorithms and strategies.
