Microcirculatory function monitoring at the bedside—a view from the intensive care

Abstract: Microcirculatory dysfunction plays a key role in the pathophysiology of various disease states and may consequently impact patient outcome. Until recently, the evaluation of the microcirculation using different measurement techniques has been mostly limited to animal and human research. With technical advances, microcirculatory monitoring nowadays becomes more and more available for application in clinical praxis. Unfortunately, measurements within the microcirculation are mostly limited to easily accessible s… Show more

“…Third, microvascular function is strongly tissue-dependent. 54 In this systematic overview, we searched for all available prospective population-based studies that focused on any marker of microvascular function. Probably because of the difficulties of microvascular measurements in population-based studies, we only found studies investigating the microcirculation in skin, eye, and kidney.…”

“…ScvO 2 can be a helpful global indicator; however, monitoring microcirculatory perfusion at the end-organ level is not readily available yet. 40 When compromised perfusion progresses to the point of eventual acidosis, organ damage most likely is occurring, even when blood pressures are normal. 1 The complexity of these changes defies overreliance on parameters such as blood pressure.…”
