Microgallbladder: Self-Remitting Acute Cholecystitis-Like Condition Unique to Patients with Cystic Fibrosis

Abstract: Microgallbladder is a nonsurgical medical condition characterized by chronic inflammation and atrophy of the gallbladder, which is considered a highly specific imaging finding unique to patients with cystic fibrosis (CF), and has been incidentally reported on abdominal imaging in up to 45% of cases with CF. The impairment of exocrine water efflux in CF leads to the production of hyperviscous biliary secretions, cholestasis, and transient cystic duct obstruction of the microgallbladder causing microcholecystiti… Show more

“…Micro-gallbladder is associated with CF and rarely with other conditions like a-1 antitrypsin deficiency, idiopathic neonatal hepatitis and biliary atresia. [1][2][3][4] Up to one-third of patients with CF have gallbladderrelated abnormalities. [1][2][3] Chronic calculous cholecystitis may also cause recurrent inflammation and severe fibrosis resulting in atrophy of gallbladder.…”

“…Non-visualisation of the gallbladder on initial ultrasound scan raised the suspicion of agenesis or intrahepatic gallbladder; thus an MRCP was carried out to better visualise the biliary tree. 4,5 No repeated episodes of acute cholecystitis leading to fibrosis and contracture of the gallbladder could be justified by her history or from morphology or histopathology of gallbladder specimen.…”

“…At US examination, the hypoplastic gallbladder seems to be contracted, collapsed or simply small in size. The microgallbladder, typically associated with cystic fibrosis, is defined as a gallbladder < 2-3 cm long and < 0.5-1.5 wide [196]. -Septated and multiseptated gallbladder.…”
