Microsurgical anatomy of the dorsal clinoidal space: implications for endoscopic endonasal parasellar surgery

Abstract: OBJECTIVE The clinoidal venous space dorsal to the internal carotid artery (ICA) has not been well studied given its inaccessibility due to obstruction by the ICA during transcranial surgery. The evolution of endoscopic endonasal surgery has provided a new perspective into the clinoidal space and a new route for paraclinoidal lesions. Understanding the dorsal clinoidal space (DCS) is vital in planning and performing endoscopic endonasal surgery in the parasellar region. A detailed and precise description of th… Show more

“…As reported, ERBB receptor tyrosine kinases play a key role in both normal physiology and cancer. Many epithelial tumors contain mutations of ERBB2, and clinical studies indicate that they are correlated with tumor progression (Hynes and MacDonald, 2009;Xu et al, 2022). When cells are exposed to different stress signals, their p53 signaling pathway is activated, activating several transcriptional programs, including cell cycle arrest, senescence, DNA repair, and apoptosis, leading to tumor growth inhibition (Marei et al, 2021).…”

“…Although this technique has been previously proposed, [9][10][11] the current study provides a detailed anatomic analysis and step-by-step maneuvers, such as the sequential removal of the PPsb as the medial and posterior wall of the foramen lacerum and the lingual process as the lateral wall of the foramen lacerum. Freeing the superior anchor of the ICA involves incising the proximal dural ring and the caroticoclinoid ligament, 18 and is necessary to further mobilize the parasellar ICA.…”

“…He underwent an endoscopic endonasal approach, and the pterygosphenoidal triangle was drilled out to expose the inferior-most aspect of the tumor residing near the foramen lacerum. Tumor was removed from the clinoidal space 11 as well as the medial wall and inferior and superior compartments of the cavernous sinus. Exposure of the foramen lacerum facilitated removal of tumor burden in the carotid canal toward the foramen lacerum.…”
