Microvascular surgery utilizing the endoscope as the sole source of visual assistance

Abstract: With the introduction of endoscopy to surgery, it has become apparent that magnification similar to the magnification provided by the surgical microscope can be achieved with its use. Endoscopic techniques provide both magnification and the ability to operate at a distance, potentially increasing the applications of microsurgery. An endoscopic unit is significantly less expensive than the operating microscope. Furthermore, it enables invasive techniques utilizing smaller incisions. The purpose of this project … Show more

“…1,[3][4][5] They all reported the possibility of performing vascular microsurgery in an open operative field with visual on-screen assistance, either with endoscopes, video microscopes, or threedimensional video imaging. No one has reported a trial to perform microsurgery in a closed operative field (optical cavity).…”

“…Jain et al 3 in 1998 concluded that no endoscopically assisted anastomosis was performed in an open field in less than 35 minutes. In our series, we spent less time, with a mean time of 27.7 minutes for the vascular anastomoses and a mean of 19.2 minutes for the neural anastomoses.…”

“…We wrote a paper years ago indicating that microvascular surgery could be done with the endoscope. 10 That is something that I think we need to reevaluate to save our necks because one of the professional hazards for us is neck problems. Now with a Da Vinci robot, we can do the vascular anastomosis very easily, but it is very expensive and cumbersome.…”

“…Furthermore, it enables invasive techniques utilizing smaller incisions. 2 The main advantages of the endoscope are the lack of bulk, almost unrestricted mobility, sterility, and the absence of necessity for eye accommodation. Moreover, the endoscope is a versatile and cost-effective tool that can be used by both plastic surgeons and other surgeons.…”
