Midface-Lifting: Evolution, Indications, and Technique

Abstract: The youthful face is often defined by malar and lateral cheek fullness with associated submalar concavity, giving a smooth contour between the different subunits coupled with an aesthetically pleasing convex lower eyelid-cheek continuum. This article reviews the key anatomical concepts of midfacial aging, the evolution of midface-lifting techniques, and indications and contraindications for their use.

“…Most experts agree that aging of the midface occurs because of increased tissue laxity with concurrent volume loss of the malar and suborbicularis oculi fat pads. [9][10][11] To more aggressively address midface aging, the extended-SMAS rhytidectomy approaches were developed, which release the SMAS attachments to lift the midface along with the lower face and neck. More recently, a number of minimally invasive approaches to facial aging have become available such as facial fillers, neuromodulator injections, and skin laser resurfacing.…”

“…Failure to mobilize the zygomatic and masseteric retaining ligaments tethering the SMAS layer effectively addressed the jowls but leaves the midface and nasolabial folds unaddressed. Most experts agree that aging of the midface occurs because of increased tissue laxity with concurrent volume loss of the malar and suborbicularis oculi fat pads 9–11 . “…The LTS is a natural part of the cheek-midface lift technique. The cheek-midface lift is a popular technique for facial rejuvenation in esthetic surgery ( 49 ). Also, some reconstruction indications have been described, e.g., for cicatricial ectropion after lower eyelid blepharoplasty or for severe ectropion with a negative vector ( 50 52 ).…”
