Migraine-Like Headache in Bacterial Meningitis

Abstract: The effect of sumatriptan has not been previously described in the treatment of the headache of meningitis, although this headache has similarities to migraine. This study presents the clinical features of two patients who had fulminant bacterial meningitis with migraine-like headache and who experienced no improvement in headache intensity after administration of sumatriptan 6 mg s.c. On these grounds the lack of response of this type of headache to sumatriptan is discussed.

“…In our 4 patients who underwent CSF studies, 3 had disclosed pleocytosis and all had elevated protein levels, which were similar to the results in a recent report, 13 supporting the theory of meningeal inflammation in patients with IHCP, which might result in meningitis‐like headaches. The headache profiles in patients with meningitis can present as throbbing headaches, accompanied by vomiting, phonophobia, or photophobia, similar to migraine attacks 19 . Elevated intracranial pressure was thought to be another possible headache etiology, 13 but this hypothesis is still a supposition due to inconsistent data 13,17 .…”
