Milestones in the evolution of hepatic surgery

Abstract: This paper describes the rapid evolution of modern liver surgery, starting in the middle of the twentieth century. Claude Couinaud studied and described the segmental anatomy of the liver, Thomas Starzl performed the first liver transplantations, and Henri Bismuth introduced the concept of anatomical resections. Hepatic surgery has developed significantly since those early days. To date, innovative techniques are applied, using cutting-edge technologies: Intraoperative ultrasound, techniques of vascular exclus… Show more

“…Liver surgery has developed significantly in the past decades. Comprehension about liver anatomy, development of parenchyma transection techniques, incorporation of new technologies and instruments made liver surgery a complex and effective specialty 5 , requiring structure available in tertiary centers 4 . In the last years, minimally invasive techniques were incorporated to liver surgery, adding a new set of complex operations to be learned and performed by liver surgeons 8 .…”

“…Moving ahead to modern times, the first liver resections were performed on patients with traumatic injuries resulting from combat in the seventeenth and eighteenth centuries. The first planned non-traumatic liver operation was performed in 1888 by Langenbuch [ 3 ]. The Australian surgeon Pringle introduced the maneuver for in-flow occlusion that carries his name in 1908.…”

“…Hepatic transection remained a challenge for all surgeons, for more than a century. The first scheduled hepatectomy was performed in 1888 from Carl Langenbuch [24]. Liver surgery was minimal thus, up to the 20th century, when Pringle maneuver was first presented, for bleeding control during emergency hepatic resections [14].…”
