Milrinone Therapy for Enterovirus 71-Induced Pulmonary Edema and/or Neurogenic Shock in Children

Abstract: Milrinone significantly reduced the 1-week mortality of enterovirus 71-induced pulmonary edema and/or neurogenic shock without adverse effects. Further studies are needed to determine whether milrinone might be useful to prevent progression of earlier stages of brainstem encephalitis.

“…The effectiveness and efficacy of milrinone treatment in patients with EV71-related PE has been proven in historically controlled [5] and randomized controlled studies [35]. The influence of selective PDE4 inhibitors and nonselective PDE inhibitors on the activity of immune cells, including lymphocytes, has been studied extensively [36], [37].…”

“…These include enterovirus (EV)-71-induced brainstem encephalitis [20], subarachnoid bleeding (SAB) [21], intracerebral bleeding (ICB) [22], traumatic brain injury (TBI) [23], ischemic stroke [24], epilepsy [25], subdural hematoma [26], epidural hematoma [27], cerebral or spinal surgery [28], ce- rebral mass lesion [29], or other rarer causes (Table 1). According to a study of 36 patients with SAB-triggered NPE, predictors for the occurrence of NPE in patients with SAB were World Federation of Neurological Surgeons class, total power, and normalized low-frequency component [30].…”

“…Since steroids may be beneficial for certain neurological conditions, such as multiple sclerosis or mass lesions with concomitant edema, and harmful in other conditions, such as TBI, application of steroids in NPE patients must be individualized [2]. In a recent study of 22 children with EV-71-induced brainstem encephalitis and NPE, the administration of milrinone resulted in a significantly better outcome with lower 1-week mortality than in controls receiving conventional treatment [20].…”

“…There is no specific antiviral treatment available till now. The remaining small proportion of children with HFMD progressing to potentially fatal cardiopulmonary failure requires intensive care including ventilation and newer therapy including milrinone [21]. The early onset of myoclonic jerk (during sleep in early stage and later also in awake state), truncal ataxia and wandering eyes (rotatory eye movement without fixation) are the early manifestation of fatal disease course [22].…”
