Minimal Hepatic Encephalopathy Affects Daily Life of Cirrhotic Patients: A Viewpoint on Clinical Consequences and Therapeutic Opportunities

Abstract: Minimal hepatic encephalopathy (MHE) is a frequent complication of hepatic encephalopathy (HE) and can affect up to 80% of patients with liver cirrhosis. It is characterized by the lack of obvious clinical signs and the presence of alterations detectable using psychometric or electrophysiological testing focused on attention, working memory, psychomotor speed and visuospatial ability. Ideally, each patient should be tested for this condition because, despite the absence of symptoms, it has severe repercussions… Show more

“…Liver patients often grapple with multimorbidity, meaning they contend with the presence of multiple chronic conditions simultaneously. The challenge lies not only in managing liver-related pain but also in addressing the intricacies introduced by comorbidities such as diabetes, cardiovascular disease, or renal impairment [ 10 ]. Each comorbidity adds a layer of complexity to the overall care plan, requiring a nuanced understanding of the interplay between different conditions and their respective treatments.…”

“…The challenges linked to the clinical diagnosis of grade I HE, which mainly relies on the expertise of the operator and is challenging to standardize across different centers, have led to a suggestion to combine MHE and grade I HE and classify them as covert hepatic encephalopathy (CHE) (2). The available evidence suggests that C/ MHE has the potential to not only result in the manifestation of OHE, but also negatively affect an individual's driving ability, heightening the likelihood of falls and accidents, and contributing to a decline in quality of life (3). Significantly, C/MHE has been identified as a prognostic indicator of unfavorable outcomes in individuals with cirrhosis, thereby posing a substantial public health concern on a global scale.…”
