Minimally Invasive Techniques for the Management of Adult UPJ Obstruction

Abstract: Ureteropelvic junction obstruction (UPJO) is a common congenital abnormality that often presents in adulthood. Open dismembered pyeloplasty was considered the gold standard for the management of this condition; however, recent advancements in laparoscopic and robotic surgery have dramatically shifted the landscape to more minimally invasive techniques. A literature search of ureteropelvic junction obstruction, pyeloplasty, endopyelotomy, laparoscopic pyeloplasty, robotic pyeloplasty, and microlaparoscopic pyel… Show more

“…For most of the 20 th century it was performed as an open operation, but since the end of the last century, A-H pyeloplasty has been used as a minimally invasive procedure – the first laparoscopic pyeloplasty was done in 1993 by Schuessler et al , as well as by Kavoussi and Peters, and the first robotic approach was performed in 2002 by Gettman et al [ 2 4 ]. Since then, minimally invasive pyeloplasty has become a well-studied, commonly used technique among urologists, with a success rate of over 90% [ 5 , 6 ].…”

“…7 The management of this anomaly has been well established in the literature. 8-10 Dismembered pyeloplasty has been considered the gold standard surgical intervention. 11 The new reality of blood supply after transplantation with most of the lumbar vessels being cutoff makes this option not available.…”

“…Pyelo-ureteral junction syndrome (PUJS), also known as primary hydronephrosis, is a common cause of urinary malformation in the upper urinary tract [14]. It is characterized by a narrowing of the junction between the pelvis and the ureter [1, 2, 46]. PUJS may result from an intrinsic, functional, congenital abnormality of the ureteral wall or extrinsic compression more often by a lower polar vessel [2, 3, 6].…”
