“… 8 While mirtazapine and mianserin are therapeutic agents for akathisia, they may cause akathisia on their own, and care must be taken when treating patients with akathisia using such medications. 32 , 33 In some cases, anticholinergic drugs cannot be used for treatment of akathisia due to side effects such as ileus, urinary disorders, and elevated intraocular pressure. In addition, anticholinergic drugs lead to tardive dyskinesia after long-term administration.…”

“…Common side effects include sedation, weight gain and increased appetite. Recent studies have shown that mirtazapine is useful for the treatment of AIAA [Poyurovsky and Weizman, 2001a;Wilson, 2005;Ranjan et al 2006;Poyurovsky et al 2003Poyurovsky et al , 2006Poyurovsky et al , 2014, although a few case reports of akathisia, both acute and tardive, induced by mirtazapine have been reported [Girishchandra et al 2002;Gulsun and Doruk, 2008;Ozyildirim and Kosecioglu, 2009;Markoula et al 2010]. As the effect of first-line agents for AIAA is variable, we reviewed existing literature to identify if enough evidence is available for mirtazapine.…”

“…[ 3 ] Intriguingly, this medication has been associated with occurrence of akathisia as an adverse effect. [ 4 ] Here we report the case of a patient with depressive illness who developed mirtazapine induced akathisia which was relieved by discontinuing the drug and by substituting it with another antidepressant.…”

“…It has been hypothesized that the beneficial effect of this medication in akathisia is due to 5HT2A antagonism while the α2 blockade might be responsible for akathisia. [ 3 4 6 ] Thus, even though mirtazapine is considered as a therapeutic option in antipsychotic induced akathisia, clinicians also need to be aware that it can produce this distressing extrapyramidal adverse effect.…”
