Mitochondrial Dysfunction in Heart Failure: From Pathophysiological Mechanisms to Therapeutic Opportunities

Giovanna Gallo,
Speranza Rubattu,
Massimo Volpe

Abstract: Mitochondrial dysfunction, a feature of heart failure, leads to a progressive decline in bioenergetic reserve capacity, consisting in a shift of energy production from mitochondrial fatty acid oxidation to glycolytic pathways. This adaptive process of cardiomyocytes does not represent an effective strategy to increase the energy supply and to restore the energy homeostasis in heart failure, thus contributing to a vicious circle and to disease progression. The increased oxidative stress causes cardiomyocyte apo… Show more

“…LVH is a consequence of an increased cardiac myocyte size and myocardial fibrosis associated with augmented extracellular connective tissue and matrix metalloproteinase-2 (MMP-2) activity, activation of the phosphoinositide 3-kinase (PI3K)/protein kinase B (AKT), and protein kinase C (PKC) pathways, abnormal Ca2+ homeostasis, mitochondrial dysfunction, oxidative stress, and apoptosis [ 18 , 19 , 20 , 21 , 22 ]. In such a context, neurohormonal dysfunction with overactivation of the renin-angiotensin-aldosterone system (RAAS) and the sympathetic nervous system (SNS) plays a pivotal role in growth and hypertrophic signaling.…”
