Mitral valve repair in patients with end stage cardiomyopathy: who benefits?

Abstract: MVR can be performed with low perioperative morbidity and mortality even in patients with advanced heart failure, modifying selection criteria for potential candidates may further improve long term outcome.

“…The mean LV ejection fraction was 23.9%, peak VO was 14, and LV end-diastolic dimension was nearly 70 mm. Remarkably, the operative mortality rate was only 1.6%, one of the lowest mortality rates of any series 8,10,12 and especially noteworthy because it represents the outcome of nearly 30 different centers. Twelve-month cumulative survival was 86.5%, and at 2 years, the cumulative survival was 85.2%.…”

“…5,6 This view was challenged by Bolling and others Ͼ10 years ago. [7][8][9][10][11][12][13] The traditional hypothesis held that the mitral valve functions as a "pop off valve" for the failing ventricle and surgical correction results in a prohibitive mortality. The Bolling hypothesis is that there is "annular solution for ventricular problem…such that reconstruction of the mitral valve annulus geometric abnormality by an undersized ring, restores valvular competency, alleviates excessive ventricular workload, improves ventricular geometry, and improves ventricular function."…”

“…6 Gummert and colleagues described a 30-day mortality rate of 6.1 % and a 5-year survival rate of 66 % in patients with dilated and ischaemic cardiomyopathy. 36 TV repair and atrial fibrillation ablation and atrial size reduction were the only accepted concomitant procedure. 6,36 Bax and colleagues reported their results for 51 patients undergoing coronary revascularisation and parallel restrictive MVRepair in ischaemic cardiomyopathy.…”

“…[11][12][13] However, the longer-term outcomes after the procedure are still largely unknown. [4][5][6][7]10 MVR is another option for cases of DCM with severe MR, [20][21][22] To a greater or lesser degree, MR coexists with severe DCM and the mechanism of MR is considered to involve tethering of the mitral leaflet by the papillary muscles, with dilatation of the mitral annulus. Mitral insufficiency leads to a vicious cycle of increasing volume overload of the dilated left ventricle, thus leading to progression of annular dilatation, worsening of MR and volume overload.…”
