Although the survival in gastroschisis has improved to more than 85%, serious morbidity with consequent long hospital stay is still very common. Alternative strategies such as planned early delivery and amniotic fluid exchange are being explored. In order to evaluate these approaches, adequate risk stratification of patients with gastroschisis is required. The aim of this study was to identify the risk factors associated with increased morbidity and mortality. A retrospective analysis of all patients with gastroschisis treated at our institution between 1988 and 2005 was conducted. Patients' data were collected from the files and compared for statistical significance with three outcome measures: (1) complicated post-operative course; (2) time to full enteral feeding (TFE); (3) length of hospital stay (LOS). SPSS was used for statistical analysis. Fifty-nine newborns with gastroschisis were admitted during the study period. Antenatal diagnosis was made in 40 cases (68%). Associated malformations (other than intestinal anomalies) were present in eight patients. One patient with trisomy 13 was excluded from the analysis. Ten patients (17%) had compromised bowel (atresia, severe ischaemia or a combination). Primary repair was successful in 47 (81%) and in 11 patients a silo was applied. Overall survival was 54/58 children (93%), four patients died. Complications associated with closure occurred in eight patients. Postoperative course was uncomplicated in 45 patients and complicated in 13. The RR for patients with compromised bowel to have a complicated post-operative course is 9 (95% CI 2-39) and the RR to die is 20 (95% CI 2-222) for these patients. Risk stratification of patients with gastroschisis appears possible based on the presence of compromised bowel before closure.

Врожденные аномалии передней стенки брюшной полости делятся на омфалоцеле и гастрошизис. Несмотря на то что они рассматриваются всегда вместе, природа этих заболеваний различна. Основные отличия представ-лены в таблице [1].Эмбриология и этиология. Брюшная стенка начинает формироваться с 4-й недели внутриутробной жизни в ре-зультате разнонаправленного роста тела эмбриона в кра-ниокаудальном и медиолатеральном направлениях. Во время 6-й недели быстрый рост печени и кишечной труб-ки вызывает перемещение средней кишки в пупочный ка-натик. В последующие 4 нед происходит процесс удлине-ния и ротации средней кишки. К 10-й неделе кишка воз-вращается в брюшную полость, где первые три отдела двенадцатиперстной кишки, восходящая и нисходящая толстая кишка прикрепляются в задней поверхности брюшной полости и перемещаются в ретроперитонеаль-ную позицию.Современное понимание происхождения омфалоцеле базируется на популярной теории, объясняющей это явле-ние как нарушение процесса возвращения органов в брюш-ную полость. Это предположение подтверждают находки в грыжевом мешке, состоящем из листков брюшины, варто-нова студня и амниона, различных внутренних органов, та-ких как печень, мочевой пузырь, желудок, яичник, яички.Происхождение гастрошизиса менее известно. Одна из теорий объясняет это заболевание нарушением разви-тия мезодермы при формировании передней брюшной стенки. Другая, более популярная теория [1], называется теорией «листков вентрального тела» (от англ. -ventral body folds). Она подразумевает, что гастрошизис форми-руется в итоге нарушения сближения латеральных лист-ков вентрального тела. Эта гипотеза подтверждается тем, что гастрошизис формируется на более ранних этапах внутриутробной жизни, чем омфалоцеле. Несмотря на то что наблюдается увеличение числа новорожденных с га-строшизисом, ни один из предполагаемых тератогенных факторов (табакокурение, низкий социальный статус, мо-лодой возраст матери) не нашел подтверждения в научных исследованиях [2-5]. ГастрошизисПренатальный диагноз. Гастрошизис встречается у 1 из 4000 появившихся на свет новорожденных [6]. Доку-ментально подтверждено увеличение числа новорожден-ных с гастрошизисом у матерей моложе 21 года [7]. По-всеместно во всем мире отмечено увеличение новорож-денных с этим заболеванием [8]. Недоношенность также существенно чаще встречается у больных с гастрошизи-сом, чем в обычной популяции детей (28% против 6%) [9].Это заболевание оказывает очевидное и значительное влияние на пациентов, их семьи и систему здравоохране-ния в целом. Новорожденные с гастрошизисом нуждаются в длительной госпитализации после рождения, которая со-ставляет от 25 до 50 дней при стоимости лечения от 70 000 до 150 000 долл. США [10][11][12][13][14]. Background: The impact of general anesthesia is considered a risk factor for developmental delay. Very few studies have been performed to measure the neurodevelopmental outcome of patients with selected malformations. Objectives: The purpose of this ambidirectional cohort study was to measure the neurodevelopmental outcome of patients with congenital gastrointestinal-tract malformations (GIM). Methods: Forty patients with relevant congenital GIM born in the period from June 2008 to April 2011 were identified. The inclusion criteria were a gestational age >32 completed weeks and surgery that required a general anesthetic within the first 28 days of life. The neonatal characteristics and anesthesia data were retrospectively collected. Based on information about the neonatal characteristics and socioeconomic background, a matched pair was found. All participants were tested at the corrected age of 24 months with the Bayley Scales of Infant Development II assessment. Results: The outcome was split into the psychomotor index (PDI) and mental developmental index (MDI). The patient group achieved a mean PDI of 103 and the control group achieved 106, i.e. these values were not significantly different. The mean MDI was 102 in the patient group and 110 in the control group. This difference was significant (p = 0.022). Detailed analysis of the items showed no significance for nonverbal items but a significant difference for verbal items (p = 0.029). Further analysis showed no correlation between relevant anesthesia data and the neurodevelopmental outcome. Conclusions: We found lower MDI scores due to worse verbal abilities in the patient group. Children born with GIM should be considered a risk group with respect to language development.
