The paper aims to describe a new technological and organizational approach in order to manage teleconsultation and telemonitoring processes involving a Physician, who remotely interacts with one or more Specialists, in order to evaluate and discuss the specific clinical conditions of a patient, based primarily on the sharing of digital clinical data, reports and diagnostic images. In the HINT project (Healthcare INtegration in Telemedicine), a teleconsultation and telemonitoring cloud platform has been developed using a Hub and Spoke architecture, based on a Business Process Management System (BPMS). The specialized clinical centres (Hubs) operate in connection with the territorial hospital centres (Spokes), which receive specific diagnostic consultations and telemonitoring data from the appropriate Specialist, supported by advanced AI systems. The developed platform overcomes the concepts of a traditional and fragmented teleconsultation and consequently the static organization of Hubs and Spokes, evolving towards an integrated clinical workflow management. The project platform adopts international healthcare standards, such as HL7 FHIR, IHE (XDS and XDW) and DICOM for the acquisition and management of healthcare data and diagnostic images. A Workflow Management System implemented in the platform allows to manage multiple and contemporaneous processes through a single platform, correctly associating the tasks to the Physicians responsible for their execution, monitoring the status of the health activities and managing possible clinical issues.

Knowledge‐intensive processes (KiPs) have emerged characterizing tasks that are data‐driven, change dynamically and depend on the knowledge and experience of knowledge workers. Case Management Model and Notation (CMMN) language was introduced for modeling unstructured, agile human‐centric processes. As the available Business Process Modeling (BPM) methods and practices are not adequate for modeling KiPs, this paper presents an empirical study of the applicability of CMMN to modeling KiPs. After experienced modelers designed the models of three KiPs scenarios utilizing CMMN, their perceptions were collected assessing the support of CMMN to the KiPs characteristics. Specifically, the modelers assessed the support of the newly introduced CMMN notation elements to the KiPs characteristics. Three of them, that is, knowledge‐driven, unpredictable, and goal‐oriented, were highlighted as the ones that get the greater support by CMMN when it comes to modeling KiPs; the participating modelers suggested that the applicability of CMMN to modeling KiPs is mostly reinforced by the Sentries element. Considering that in a knowledge‐driven economy, organizations are called to transform their way of thinking and manage KiPs, we value the modelers' opinions since they shall apply such practices in the future. This research is a first step to assess whether CMMN may be a “fit” for modeling KiPs. When considering the potential of CMMN for KiPs modeling, the perceptions of such real CMMN users are also first indications of the CMMN future in terms of application.

Knowledge work is growing in today's marketplace, supported by "knowledge workers," a valuable asset in organizations. Traditional software applications are too rigid to support knowledge work or empower knowledge workers to perform their nonroutine tasks. Therefore, most knowledge work today is managed manually or using ad-hoc technologies such as e-mail and social media. Adaptive case management (ACM) is a new promising approach for effectively supporting knowledge work, empowering knowledge workers, and overcoming the limitations of traditional software applications. Nevertheless, ACM is an emerging field accompanied by confusion and debate among researchers and practitioners. This study aims at providing an overview of the key concepts and principles of ACM. Furthermore, this study outlines the major differences between traditional business process management and ACM.
