Modeling hospital infrastructure by optimizing quality, accessibility and efficiency via a mixed integer programming model

Abstract: BackgroundThe majority of curative health care is organized in hospitals. As in most other countries, the current 94 hospital locations in the Netherlands offer almost all treatments, ranging from rather basic to very complex care. Recent studies show that concentration of care can lead to substantial quality improvements for complex conditions and that dispersion of care for chronic conditions may increase quality of care. In previous studies on allocation of hospital infrastructure, the allocation is usually… Show more

“…In its informal opinion, the ACM assumed that patients were willing to travel further for better care for complex procedures. This argument is partly supported by the relevant academic literature: it has been shown that some patients are more inclined to travel further for higher quality of care, to a certain extent at least (Tanke and Ikkersheim, 2012;Ikkersheim et al, 2013;Gutacker et al, 2016;Middelveldt et al, 2018;Vallejo-Torres et al, 2018).…”

“…Key tools include linear programming, including integer and mixed-integer programming, and simulation models. [12][13][14][15][16][17] Operations researchers and industrial engineers, in collaboration with administrative decision-makers, can use these tools to develop models that represent real-world scenarios. Such models can have meaningful results that inform operationally feasible decisions.…”

“…It is worth mentioning that the use of infrastructure, understood as the number of beds, the number of diagnostic equipment and the number of operating theatres, are very often used in studies on the efficiency of the health care system [Rohrer, 1988;Sultan, Crispim, 2018;Büchner et al, 2016]. An interesting look at the medical infrastructure is presented by David Ikkersheim and his team [Ikkersheim, 2013]. They examine the degree of concentration of medical infrastructure in the context of the quality and effectiveness of care services in chronic diseases (general practitioners) and comprehensive and acute care (hospital).…”
