Modern clinical and fundamental aspects in the diagnosis and treatment of patients with ischemic cardiomyopathy (Review)

Show more

“…Since the first successful LV reconstruction using CPB by Dor et al in ICM patients in the mid-1980s, a large number of similar procedures have been performed around the world, and a number of minor modifications to this procedure have been introduced. 2,3 The concept of LV surgical reconstruction is based on the exclusion of scar tissue and reduction of the LV to a more physiological shape and volume, which improves LV function. 2 It is well known that patients undergoing LV reconstruction are at high surgical and cardiac risk; therefore, the main treatment outcome of interest is still early mortality and subsequently long-term survival and quality of life in this group of patients.…”

“…2 Clinically, this pathology is symptomatic in a significant decrease in left ventricular (LV) ejection fraction (EF), heart chamber dilatation, and the presence of atherosclerotic lesions of the coronary arteries. 3,4 The disease is characterized by progressive LV remodeling, thinning of its walls with an aneurysm formation, impaired cardiac hemodynamic parameters, and changes in myocardial morphology at the microscopic and ultrastructural levels. 5 The severity of ICM depends on the degree of morphological changes in the ventricles and is associated with the so-called process of cardiac remodeling.…”
