Abstract

za hirurgiju Kliničko bolnički centar Zemun -Beograd, 2 Kliničko bolnički centar "dr dragiša Mišović" Klinika za hirurgiju, Beograd, 3 Institut za onkologiju i radiologiju srbije, služba hirurgije, Beograd, Apstrakt savremeni svetski stavovi podrazumevaju limfadenektomiju kao terapijsku proceduru koja je integralni deo radikalne hirurgije karcinoma želuca. Ona se izvodi u pokušaju da se hirurgija malignoma uči-ni što radikalnijom. Cilj svake hirurške procedure je potpuno uklanjanje tumora i svakog tkiva koje njime može biti zahvaćeno. Težnja radikalne hirurgije je u eradikaciji maligne bolesti, a limfadenektomija bitno doprinosi ovom cilju. Limfadenekotmija kod gastričnog karcinoma podrazumeva disekciju limfnih nodusa prve, druge grupe, što se smatra standardnom limfadenektomijom d2. Pored hirurškog načina lečenja karcinoma želuca, koji je ostao dominanatan, u poslednje vreme se razvijaju novi modaliteti leče-nja želuca. I pored isticanja u prvi plan značaj hirurškog lečenja i sistematske limfadenektomije, kriterijumi koje je dalo japansko udruženje za gastrični kancer od 2010g. razlikuje i druge modalitete leče-nja gastričnog karcinoma. Karcinome želuca može-mo tretirati: endoskopski, laparoskopski, klasičnom hirurškom resekcionom operacijom, hemioterapijom (koja može biti preoperativna i postoperativna) i palijativnom hirurgijom. svaki od ovih modaliteta leče-nja utiče na bolji ishod bolesti pacijenata. Izbor adekavtnog tretmana pacijenta sa karcinomom želuca zavisi od stepena uznapredovalosti maligne bolesti. AbstractThe modern world views include lymphadenectomy as a therapeutic procedure that is an integral part of radical surgery of stomach cancer. It is performed in an attempt to make the malignant surgery more radical. The aim of any surgical procedure is complete removal of the tumor and any tissues that may be affected by it. The pursuit of radical surgery in the eradication of malignant disease, a lymphadenectomy significantly contributes to this aim. The lympadenetomy in gastric cancer involves lymph node dissection first, second group, which is considered the standard d2 lymphadenectomy. In addition to surgical therapy of gastric cancer, which remains dominanatan, lately developing new modalities of treatment of stomach. Besides highlighting the fore the importance of surgical treatment and systemic lymphadenectomy, the criteria provided by the japanese association of gastric cancer than 2010g. knows and other modalities of treatment of gastric cancer. Gastric cancer can be treated: endoscopic, laparoscopic, conventional surgical resection surgery, chemotherapy (which may be the preoperative and postoperative) and palliative surgery. Each of these modalities of treatment influenced the better outcome of patients.
