Mohs Micrographic Surgery: Development, Technique, and Applications in Cutaneous Malignancies

Abstract: Mohs micrographic surgery (MMS) is a specialized technique for treating skin malignancies that offers the highest cure rate by allowing histological evaluation of the entire peripheral and deep margins. MMS also maximally preserves as much uninvolved, normal adjacent tissue as possible, allowing for the best cosmetic and functional outcomes. When used for appropriate indications, this technique is also more cost-effective than other treatment modalities. In this article, the authors will discuss the developmen… Show more

“…1 Mohs micrographic surgery (MMS) is a surgical technique that allows evaluation of the entire peripheral and deep margins of excised skin malignancies, leading to higher cure rates, compared to conventional excision. 2 MMS also preserves the maximum normal adjacent tissue possible, resulting in smaller resection defects, and this can be especially important in aesthetic and functional areas, as in the face. 1,2 MMS represents an option for different types of skin malignancies, ranging from basal cell carcinoma, squamous cell carcinoma, atypical fibroxanthoma, dermatofibrosarcoma protuberans, Merkel cell carcinoma, microcystic adnexal carcinoma, sebaceous carcinoma, eccrine porocarcinoma, leiomyosarcoma, extramammary Paget's disease, aggressive digital papillary adenocarcinoma and even melanoma.…”

“…Therefore, it is necessary for the Mohs surgeon to be an educated dermatopathologist at the same time [9]. This method achieves the best results in the treatment of non-melanoma skin cancers, especially in regions where it is important to preserve function and/or cosmetic results, such as the maxillofacial region [10]. Cryosurgery in the treatment of skin cancer is limited to well-limited, low-risk superficial lesions that are smaller than 20 mm in diameter [11].…”

“…Zbog toga je neophodno da Mohs hirurg istovremeno bude edukovan dermatopatolog [9]. Ovom metodom se postižu najbolji rezultati u lečenju nemelanomskih karcinoma kože, a naročito u regijama gde je važno očuvati funkciju i/ili kozmetski rezultat, kao što je maksilofacijalna regija [10].…”

“…Definitive treatment of AMHN includes surgical resection. For melanomas in situ , Mohs surgery is the most cost‐effective, tissue‐conserving option 18–20 ; however, for invasive melanomas, a wide local excision is recommended. The recommendation for the surgical excisional margins of AMHN follows the same principles suggested by the National Comprehensive Cancer Network clinical guidelines 21,22 .…”
