Molecular mechanisms of glucocorticoids action: implications for treatment of rhinosinusitis and nasal polyposis

Abstract: Intra-nasal glucocorticoids are the most effective drugs available for rhinosinusitis and nasal polyposis treatment. Their effectiveness depends on many factors and not all of them have been well recognized so far. The authors present the basic information on molecular mechanisms of glucocorticoid action, direct and indirect effects of glucocorticoids on transcription of genes encoding inflammatory mediators. They focus on recently proved nongenomic mechanisms which appear quickly, from several seconds to minu… Show more

“…Previous studies have proven that topical steroids are effective for both CRSwNP and CRSsNP (evidence level I). The clinical efficacy of corticosteroids may depend in part on their ability to reduce the secretion of chemotactic cytokines from nasal mucosa and polyp epithelial cells …”

“…Both topical and systemic glucocorticoids may affect the eosinophil function by both directly reducing eosinophil viability and function or indirectly reducing the secretion of chemotactic cytokines by nasal mucosa and polyp epithelial cells. [12] Systemic steroids are reserved for advanced or refractory cases particularly when allergy is present and it results in relatively rapid short-term dramatic improvement, nasal symptoms and endoscopic findings (medical polypectomy). Simple saline nasal douching to help cleanse the nose prior to topical medications is beneficial as it improves nasal mucociliary clearance.…”

“…The mechanisms of action of corticosteroids in CRSwNP have been previously reviewed in the literature [32,33]. Corticosteroid's mechanisms involve corticosteroid receptors, which have been described to be present in all the nasal cell types with greatest numbers in epithelial cells, submucosal glands and leukocytes [34].…”
