Molecular Surveillance of Carbapenem-Resistant Gram-Negative Bacteria in Liver Transplant Candidates

Abstract: Background: Carbapenem-resistant Gram-negative bacteria (CRGN) cause life-threatening infections due to limited antimicrobial treatment options. The occurrence of CRGN is often linked to hospitalization and antimicrobial treatment but remains incompletely understood. CRGN are common in patients with severe illness (e.g., liver transplantation patients). Using whole-genome sequencing (WGS), we aimed to elucidate the evolution of CRGN in this vulnerable cohort and to reconstruct potential transmission routes.Met… Show more

“…It was reported that the CRKP carriage rate was 7% in ICUs in Israel and 28% in China [ 22 , 23 ]. To the best of our knowledge, research investigating CRE colonization in ESLD patients is mainly conducted in Germany [ 19 , 24 ]. Our study showed that the CRE colonization rate in ESLD patients was 5.5%, which was higher than the 2.8% in liver transplant candidates in Germany [ 24 ].…”

Although bioinformatic pipelines are not in broad use in routine clinical microbiology, they are getting more and more involved in scientific infection control measures (Schultze et al 2021 ) and are expected to gain access to routine laboratories soon.…”

“…Particularly noteworthy is the MICU's focus on liver transplantation, which is reflected in the high number of patients on the waiting list for liver transplantation with acute decompensation of disease and acute-on-chronic liver failure. Infections in patients with cirrhosis represent an increasing health and economic burden [23,24]. Patients with liver cirrhosis are prone to acute decompensation, acuteon-chronic liver failure (ACLF), and hospitalization, including the application of broadspectrum antibiotics due to infections.…”
