Molecular Typing of Pneumococci for Investigation of Linked Cases of Invasive Pneumococcal Disease

Abstract: In winter 2007-2008, an outbreak of pediatric pneumonia caused by serotype 5 pneumococci was identified in a northeast London suburb. Variable number of tandem repeat analyses clustered these pneumococci from the other serotype 5 pneumococci in the United Kingdom, highlighting the importance of this discriminative typing method in supporting epidemiological investigations.

“…An MLVA scheme has been developed for S. pneumoniae that scores electrophoretically-determined size variation in tandem repeat arrays from 16 loci (Koeck et al, 2005). This MLVA scheme has been useful for confirming relatedness of pneumococcal outbreak isolates of serotypes 1 (Yaro et al, 2006) and 5 (Pichon et al, 2010). However, use of MLVA to further elucidate population structure can be limited by the independent evolution of the same repeat array sizes in different strains (i.e.…”

“…In the first description, the Colombia 5 - ST289 clone showed resistance to co-trimoxazole, tetracycline, and chloramphenicol (26). This Ser5 clone has been previously found as a cause of IPD in children from Israel (25) and in outbreaks in Canada and the United Kingdom (9,12). However, in Israel, the ST289 clone was susceptible to all antimicrobials studied, with the exception of co-trimoxazole (36% resistant) (25).…”

“…Ser5 pneumococci have been associated with chains of infection, such as those described in Israel (11), the United Kingdom (12), and Canada (9,13,14). Outbreaks of pneumococcal infections have been documented and occur typically in closed communities within institutions, and they tend to be caused by a limited number of pneumococcal serotypes.…”

“…Although deaths from pneumonia in children in the UK are rare, there is an increasing observation of empyema and complicated pneumonia developing 53. The explanation for this is not as yet clear but could be related to the changing pneumococcal serotype prevalence54 which was expanded on by Professor D Goldblatt (London). Professor Goldblatt's talk on the pneumococcal vaccines centred on the introduction of new pneumococcal conjugate vaccines and how these have led to a reduction in the incidence of invasive pneumococcal disease.…”
