This study aimed to establish factors associated with delayed surgery in patients with proximal femoral fracture and to assess patients’ health-related quality of life (HRQoL) after surgery including all-cause 6-months mortality. This was a single-center, observational, prospective cohort study that included patients with a proximal femur fracture. We described patients’ HRQoL measured by EuroQoL (EQ-5D-5L and EQ-VAS) questionnaire and perioperative complications (including mortality) 6 months after surgery. We included 163 patients with a mean age of 80.5 years, the majority were women and 76.1% reported falling from their own height. The mean time between hospital admission and surgery was 8.3 days (SD 4.9 days) and the mean hospital stay was 13.5 days (SD 10.4 days). After adjustment, the principal factor associated with delayed surgery was adjournment in surgery authorization (3.7 days). EQ-5D-5L index values and the VAS score at 1 month after surgery were 0.489 and 61.1, at 3 months were 0.613 and 65.8, and at 6 months 0.662 and 66.7 respectively. Mortality at 6 months of follow-up was 11% (18 patients). In conclusion, administrative authorization was the strongest associated factor with delayed time from hospital admission to surgery. HRQoL of patients with a proximal femoral fracture improved 6 months after surgery.Trial registration: NCT04217642.

“…Mientras que en nuestro estudio la mediana indica que el 50% de los pacientes tienen un intervalo de cirugía por encima de los 4 días y el resto por debajo de este valor debido a la amplia variabilidad de tiempos encontrados. Se encontró que 19 de los 175 pacientes estudiados fueron intervenidos luego de 12 días, además, rangos entre los 1 a 38 días para los hombres y 1 a 27 días para las mujeres (22). En otro estudio realizado en México por Hernandez, Mejía, Benítez estudiaron 84 pacientes con fractura intertrocanterica de los cuales se encontraron 43 (51.11%) con diabetes mellitus, 32 (38.09%) con hipertensión arterial sistémica, 9 (10.71%) con enfermedad isquémica del corazón, 8 (9.52%) con insuficiencia renal crónica, lo que guarda similitud con nuestro estudio en donde se evidenció que la comorbilidad más frecuente es la hipertensión arterial y la diabetes mellitus (23).…”

Introducción: Las fracturas del fémur proximal, también llamadas fracturas de cadera, se encuentran entre las principales causas de morbilidad en el mundo donde la población más afectada son los adultos. Esto afecta en gran medida a la salud pública debido a su alto costo socio-económico y la incapacidad laboral que genera. Objetivo: Determinar la epidemiología de fractura de cadera en el adulto atendido en un hospital de tercer nivel de Cúcuta, Norte de Santander, Colombia. Materiales y métodos: Un estudio descriptivo, transversal, de análisis retrospectivo, con enfoque cuantitativo. Se tomaron 175 pacientes con fractura de cadera, que cumplían con los criterios de inclusión, con una población diana de mayores de 50 años de edad. Adicionalmente de las variables que se registraron: Edad, género, causa, comorbilidad, clasificación, intervalo de cirugía, tipo de intervención quirúrgica. Resultados: De los 175 pacientes se encontró una edad media general de 75.35 años, el 60% corresponde a pacientes de sexo femenino, la causa más común son los factores extrínsecos, la mayoría de pacientes no padecía ninguna enfermedad, en mayor relevancia acudían por una fractura intertrocanterica. El 50% de los pacientes son intervenidos quirúrgicamente en un periodo de tiempo superior a los 4 días. Conclusiones: A partir de los hallazgos encontrados, se acepta que la frecuencia de fractura de cadera se encuentra asociada a diversas variables las cuales fueron analizadas en este estudio obteniendo resultados novedosos que quedan a disposición de la academia en bienestar de la salud, impulsando nuevos estudios.
