Morning Home Blood Pressure Is a Strong Predictor of Coronary Artery Disease

Abstract: Morning HBP is a strong predictor of future CAD and stroke events, and may be superior to clinic BP in this regard. There does not appear to be a J-curve in the relationship between morning HBP and stroke or CAD events. (Home blood pressure measurement with Olmesartan Naive patients to Establish Standard Target blood pressure Study [HONEST]; UMIN000002567).

“…During a mean follow‐up of 4 years, morning SBP measured at home was shown to improve the discrimination of incident stroke beyond traditional risk factors that included clinic SBP, whereas the addition of evening SBP reduced the discrimination of incident stroke. Although neither morning nor evening SBP improved risk prediction of CAD in the J‐HOP study, a larger study of more than 21 000 patients with treated hypertension did find a strong correlation between morning home BP and future CAD and stroke events . Moreover, the data suggested that morning home BP might be superior to clinic BP in this regard.…”

“…28,30,31,33 The results of the prospective J-HOP study clearly demonstrated that patients with wellcontrolled morning home SBP (<135 mm Hg at baseline) were at lower risk of having a stroke than those with a higher morning home SBP. 53 Furthermore, the home BP-guided interventional HOMED-BP study demonstrated that achievement of lower home SBP was significantly associated with better cardiovascular prognosis. 53 Furthermore, the home BP-guided interventional HOMED-BP study demonstrated that achievement of lower home SBP was significantly associated with better cardiovascular prognosis.…”

“…Morning systolic home BP associated with minimum risk was 124 mm Hg. Additionally, a recent analysis from the HONEST study demonstrated that morning systolic home BP ≥ 155 mm Hg was associated with an HR of 6.24 (95% CI, 2.82 to 13.84) for predicting future coronary artery disease compared with an HR of 3.51 (95% CI: 1.71-7.20) for clinic SBP of ≥160 mm Hg [21].…”
