Resumo Fundamento As pontes miocárdicas (PM) são anomalias anatômicas com possíveis repercussões clínicas, e, portanto, seu entendimento merece atenção. Objetivo Para determinar a prevalência e caracterizar a PM em corações humanos do estado do Ceará. Métodos: Foram usados cinquenta corações de cadáveres humanos adultos da Faculdade de Medicina da Universidade Federal do Ceará, Brasil. Os corações foram dissecados para identificar PMs que passam sobre parte da artéria coronária. O segmento da artéria (proximal, médio e distal) com a ponte foi identificado. O diâmetro externo da artéria nos pontos proximal e distal da PM foi medido. O comprimento e a espessura da PM também foram medidos com um calibre eletrônico. O índice de massa muscular (IMM) da PM foi calculado como o produto do comprimento pela espessura expresso em milímetros. O nível de significância adotado para a análise estatística foi 5%. Resultados A PM foi confirmada em 40% da amostra. Aproximadamente um terço da amostra tinha apenas 1 PM. A PM foi encontrada mais frequentemente sobre o ramo interventricular anterior da artéria coronária esquerda (59,25%, p = 0,02), e sua prevalência em outros ramos foi muito mais baixa (22,23%). Os segmentos das artérias mais afetados foram o superior (44,44%) e o médio (40,74%). O diâmetro médio das artérias proximais em relação à PM foi de 2,38 ± 0,97 mm (intervalo = 0,78 - 5,15 mm), e o diâmetro distal da PM foi de 1,71 ± 0,75 mm (intervalo = 0,42 - 3,58 mm). O comprimento foi medido como média = 8,55 ± 5,27 mm, e a espessura média foi de 0,89 ± 0,33 mm. Conclusão A alta prevalência de PM tem mais probabilidade de afetar o sistema da artéria coronária esquerda com IMM maior do que outros ramos afetados.

Coronary dominance describes the anatomic variation of coronary arterial supply, notably as it relates to perfusion of the inferior cardiac territories. Differences in the development and outcome in select disease states between coronary dominance patterns are increasingly recognized. In particular, observational studies have identified higher prevalence of poor outcomes in left coronary dominance in the setting of ischemic, conduction, and valvular disease. In this qualitative literature review, we summarize anatomic, physiologic, and clinical implications of differences in coronary dominance to highlight current understanding and gaps in the literature that should warrant further studies.

Background: Myocardial bridging (MB) is a congenital coronary anomaly that can lead to harmful cardiac consequences if not diagnosed and treated. Objectives: This study aims to evaluate the Epidemiological, angiographic, and cardiovascular outcomes of patients diagnosed with myocardial bridge. Methods: The current research was conducted as a retrospective study in which 151 patients who underwent angiography participated. These patients were admitted between May 2014 and May 2016 at Heshmat and Aria hospitals in Rasht. The collected information in the present study included age, gender, blood pressure history, diabetes mellitus, blood lipid disorders, ischemic changes in ECG, ischemic changes in heart scan, output fraction, ischemic changes in exercise test, number of bridges, name of vessel, length of muscle bridge, and distance from the opening. To analyze the variables, the Chi-square test and independent t-test were used. Results: As a result, we found that myocardial bridging was present in 57 patients, and 95.08% of the bridges were located above LAD. According to this study, there is no significant relationship between the length of the bridge and the distance of the bridge from the vessel opening with the occurrence of ischemic changes in ECG and myocardial perfusion scan (P > 0.05). However, the average distance of the bridge from the vessel opening in patients who had a positive exercise test was lower compared to the patients who had a negative test. They had a negative exercise test, and this difference was statistically significant (P < 0.05). Conclusions: Furthermore, in patients diagnosed with an MB by CAG, the most MB located in the LAD, and this issue can help to better understand the outcome and how to manage these patients.
