Foramen magnum is one of the several apertures in the base of the skull, through which medulla oblongata and other vital structures are transmitted. Knowledge on the normal anatomy of the base of the skull, especially the foramen magnum and its associated structures, is important to correlate its morphometric features with relevant clinical practices and may be in sex determination. Some anatomical features and gender differences in the human cranium are unique to each population and may be influenced by genetic, environmental and socio-demographic factors. In the present study morphometry of foramen magnum and its variation with sex and age was evaluated. The anteroposterior diameter (APD), transverse diameter (TD), and the area of foramen magnum (AFM) of each CT scan images of individuals who had head computed tomography scan at Tikur Anbessa Specialized Hospital, Addis Ababa, were measured by using institutional picture archiving and communication system workstation through institutional MedWeb. The overall APD of FM was 34.55 ± 3.21mm, TD was 28.48 ± 3.03mm and AFM was 814.77 ± 126.52mm2 in the present study. The mean of measurements of all the three evaluated parameters of foramen magnum was larger in males and the differences were statistically significant at P ˂ 0.05 between sexes. However, no statistically significant (P > 0.05) difference wasdetected in all the measured parameters among different age groups. In conclusion, the result of our study has reinforced the idea of likelihood of determining sex by evaluating the morphology of foramen magnum when essential.

Objectives: The structures passing through the foramen spinosum and its neurovascular relationships are of great importance for surgical approches directed to middle cranial fossa. The aim of the present study was to examine the number and location of the foramen spinosum (FS) in 3D-CT images. Methods: The study was retrospectively conducted on 3D-CT images of 177 adults. Firstly, the transverse section passing through the upper edge of the orbit, extending parallel to the Frankfurt plane was chosen. Then, the x and y-axes were determined on that transverse section. The coordinates, number, and location of the FS with respect to the foramen ovale (FO) were identified accordingly on x and y-axes. Results: While 1 FS was present in 90.96% of a total of 354 sides of 177 heads, there were 2 FS and 3 FS in 8.76% and 0.28% of the sides, respectively. The FS was located posterolaterally in 97.68%, posteriorly in 2.06%, and laterally in 0.26% with respect to the FO. In terms of FS coordinates, there was no statistically significant difference between gender and sides in the distance of the FS to the x-axis, but there was a statistically significant difference between gender and sides in the distance of the FS to the y-axis. Conclusion: Evaluation of the number of the FS and its location would help identifying and preserving neighbouring neurovascular structures during surgical interventions directed to the middle cranial fossa.

Background: This study investigated the shapes and dimensions of the foramen ovale (FO) and foramen spinosum (FS) in a specific population sample from Delta State, Nigeria, emphasizing their significance in various neurosurgical procedures. Materials and Methods: Following ethical clearance, this retrospective study analyzed 250 brain computed tomography (CT) images from the Radiology Department of a teaching hospital in Delta State, Nigeria. The sample included images from 100 females and 150 males, all aged 18 years or older. The dimensions of the FO and FS were measured on axial sections using a standardized protocol. Statistical analyses were conducted with Statistical Package for Social Sciences (IBM, Chicago, Illinois, United States) software, categorizing data by gender and age groups. Inferential statistics, including Student t test, analysis of variance, and Pearson’s correlation, were performed, with findings considered significant at P < 0.05. Results: The predominant shapes of the FO and FS were oval and round, respectively. Significant side differences were observed in the length of the right FS (P = 0.040). Gender differences were noted in the length of the bilateral FO (P = 0.010) and the width of the left FO (P = 0.001). A significant correlation with age was found for the width of the left FO (P = 0.006) and the distance from the right FO to FS (P = 0.037). The FS was absent in 24 cases (9.6%). Conclusion: This study highlights age-, side-, and gender-related differences in the shapes and dimensions of the FO and FS. It emphasizes the importance of precise preoperative radiological assessments to ensure optimal outcomes for neurosurgical planning.
