Morphological Changes of Ureteropelvic Junction (UPJ) in Case of Congenital Hydronephrosis – Review of Literature

Abstract: Congenital hydronephrosis is the most common cause of antenatally detected hydronephrotic transformation. Obstruction of UPJ can be intrinsic or extrinsic. Most cases of intrinsic stenosis do not require any treatment as it disappears during growth of a child. It is considered that in these cases wave of peristalsis appears again in aperistaltic UPJ. The cells responsible for peristalsis wave impulses, mechanisms of conduction of these impulses and regulation of them are not well recognized. Similarly, it is n… Show more

“…Congenital obstruction can lead to poor urine excretion, increase the expansion pressure in the pelvis, compress the renal parenchyma, and cause changes in multiple vascular factors such as angiotensin II in the kidney, then it will cause renal vasoconstriction, promote a further increase of renal vascular resistance and decrease renal function. 10,11 Without timely treatment, congenital hydronephrosis may cause renal failure. So it is extremely important to determine the degree of obstruction and renal function in clinical practice.…”

“…Histological alteration in kidney like glomerulosclerosis, dilated tubules and dense chronic inflammatory infiltrate have also been noted in many studies. 2,3 Transport of urine from kidney to bladder is an active process based on well coordinated one way peristalsis. It is believed that contraction creating peristaltic wave in the ureter is initiated by cells with pacemaker activity in proximal part of renal calyces.…”
