Mortality and prognostic factors in connective tissue disease‐associated pulmonary arterial hypertension patients complicated with right heart failure

Abstract: Objective To identify predictive factors associated with mortality in connective tissue disease‐associated pulmonary arterial hypertension (CTD‐PAH) patients who were complicated with right heart failure (RHF). Methods In this single‐center retrospective study, baseline demographics, clinical features, laboratory results, and hemodynamic assessments were collected. Kaplan–Meier analysis was applied to analyze all‐cause mortality. Univariate and forward stepwise multivariate Cox proportional regression analyses… Show more

“…Most RHF patients who die in ICU experience sequential multiple organ failure, so increasing lactate trend is a helpful marker of worsening organ hypoperfusion in RHF and decompensated pulmonary hypertension 7,8 . Notably, our findings diverge from those of Deng et al 1 in that we did not detect a correlation between lactate levels at admission and mortality among RHF patients. Nevertheless, our study substantiates the notion that monitoring the lactate trend represents a valuable tool in comprehending the course of acute RHF.…”

“…The leading causes of death were RHF and infections. Deng et al 1 showed that lactic acidosis (lactate >2.85 mmol/L) was an independent predicting factor for mortality in patients with CTD‐PAH complicated by RHF.…”
