Motor Vehicle Crash Risk Among Adolescents and Young Adults With Attention-Deficit/Hyperactivity Disorder

Abstract: IMPORTANCE Attention-deficit/hyperactivity disorder (ADHD) often persists into adolescence, when motor vehicle crash risk peaks. We know little about when adolescents with ADHD get licensed and, once they do, the extent to which they have increased crash risk compared with adolescents without ADHD.OBJECTIVES To examine the association between ADHD and both driver licensing and crash involvement and whether it varies by sex, licensing age, and/or being prescribed ADHD medication at licensure. DESIGN, SETTING, A… Show more

“…137 Risk-taking behaviors increase in this age group, 138 which can lead to high rates of injuries, 139 teenage pregnancy, 140 and driving accidents. 141 In our practice, we find that difficulties at school are often exacerbated by increased cognitive demands, decreased external structure, and longer days. Adolescents may not adhere to or may discontinue their medication 110,142 even though it helps prevent risky behaviors and increases academic performance.…”

“…With this reduction in the symptoms required for an ADHD diagnosis in the latest DSM, there are even more drivers with ADHD on the roadways today. The characteristic impairments in attention and impulsive behavior make the cognitively-demanding task of driving difficult for individuals with ADHD [14,63,64]. Teenagers with ADHD have almost four times as many traffic citations and car crashes than their typically-developing peers, and are seven times more likely to have a second crash [63].…”

“…Predisposition to risky driving behavior and difficulties in attention regulation place individuals with ADHD at a much higher susceptibility to driving distractions than teens without ADHD [8,14]. Reimer, Mehler [8] investigated the potentially detrimental effects of talking on a cell phone on simulated driving performance of ADHD drivers between the ages of 17 and 24.…”

“…Adolescents with attention-deficit/ hyperactivity disorder (ADHD) are at higher risk for MVCs and injury. Teen drivers with ADHD experience a risk of crash 36% higher than their peers, 51 a risk that does not vary by sex, by age, or over time. Drivers with ADHD may be less attentive to driving tasks at baseline and at least as susceptible to distraction caused by technology, passengers, and external factors.…”
